2) The agency will develop an on-going training curriculum for the consumer to review when <br /> needed, including how to deal with recruiting, interviewing, hiring, managing, training, <br /> evaluating and dismissing an attendant/aide. <br />3) The consumer will be advised of the right to set the schedule of the attendant/aide, establish <br /> the wages of the attendant/aide, and instruct the attendant/aide of the tasks to be provided <br />4) The consumer will be given information about fiscal activities and an opportunity to ask <br /> questions about their role in this area. <br />5) The CAP/DA budget constraints will be reviewed to assure the consumer understands how <br /> choices made may affect other needed supplies and services. <br />6) The consumer will be informed of what constitutes adequate health, safety and well being in <br /> relation to the services established by the consumer. <br />7) The consumer will have the opportunity to have their chosen attendant(s)/aide(s) participate <br /> in an orientation given by the lead agency and financial management entity. <br />8) The consumer will be given the criteria below, which must be followed in selecting an <br /> attendant/aide. The attendant/aide must: <br /> · Be a US citizen or legal alien and have a valid Social Security Number <br /> · Be 18 years of age or older <br /> · Be able to communicate with the consumer <br /> · Sign an agreement with the consumer detailing tasks, days, and number of hours to be <br /> provided <br /> · Submit to a criminal background check <br /> · Obtain a Health Services card from the health department if requested <br /> · Not be a spouse or legally responsible guardian, such as a court appointed guardian <br />9) The consumer will be informed that a family member, friend, or neighbor may be hired to <br /> perform personal care tasks without the individual being required to meet a pre-determined <br /> set of requirements. The consumer will be responsible for training the individual to meet <br /> their needs. <br />10) The consumer will be given the opportunity to select a representative/surrogate to assist with <br /> some or all of the decision-making/training. ~ <br />11) The consumer will be advised that the representative/surrogate may not be paid to perform <br /> tasks for the consumer. <br />12) The agency will develop a monitoring system that will encourage consumer participation, but <br /> readily identify problems. <br />13) The agency will develop a consumer bill of rights. Both the consumer and the attendant/aide <br /> will be advised of their rights and responsibilities in view of their participation in the pilot <br /> project. <br />14) The consumer will be advised of their right to return to the traditional CAP/DA model at any <br /> time if not satisfied with directing their own care. <br /> <br />8. DELIVERY <br /> <br />Outreach <br />Outreach will be accomplished by: <br /> · Advertisements in the two local newspapers serving Cabarrus County <br /> · Interview on county public service television station <br /> <br /> <br />
