5. USE(S) OF NON-GRANT FUNDING FOR NEEDS OTHER THAN <br />ATTENDANT/AIDE <br /> <br />As with current participants, _we will assure that any emergency needs are addressed in working <br />through other state programsisuch as Emergency Assistance and the Crisis Intervention Program. <br />We also have a generous Getleral Assistance fund we may access for emergency, essential needs. <br />Additionally, we assure that ill persons eligible are enrolled in the Food Stamp Program. <br /> <br />The community boasts a Cabiarrus Cooperative Christian Ministry, composed of several area <br />churches. The agency has w, rked collaboratively with the ministry to assure numerous one-time <br />needs have been met. Also, llhe providers of public utilities have been sympathetic to our <br />disabled clients and worked ,,ell with us to assure public services are not turned off when a <br />financial crisis occurs. Our 1.1trgest provider of electricity has given our Medicaid clients a <br />discount on their monthly bills. In the past, we have worked with area churches to get ramps <br />installed when the CAP/DA l~udget could not accommodate these costs. Additionally, we have <br />worked closely with the Cab.in'us Department of Aging to assure our clients obtain fans and air <br />conditioners when respirator) stress and/or heart conditions were a factor. <br /> <br />The flexibility offered under i~ CDS waiver would enable the purchase of supplies and services <br />unrelated to attendant/aide c~'e, assistive devices (including but not limited to communication <br />aides, adaptive equipment), OF, er-the-counter-drugs and supplies, emergency pest control, home <br />modifications related to clie~ need, and services or goods (microwaves, washer/dryer). Often <br />our current CAP/DA clients ~re in need of these items and services, which cannot be offered <br />under traditional CAP/DA plijtns of care. <br /> <br />6. EFFECTIVE COLLABORATION ON CARRYING OUT A MAJOR PROJECT <br /> <br />Cabarms County DSS now c{ntracts with Good Health Services (GHS), a gaffing agency <br />located in Raleigh. Staff pro _~ided by GHS has served our clients well. Plans are to modify the <br />contract to enable GHS to pr4vide financial management for a nominal fee. GHS will assure that <br />payroll functions arc completed for participants of the Consumer-Directed Services model. As <br />lead agency will bill EDS for~reimbursement. <br /> <br />7. SERVICE-DELIVERY SYSTEM CHANGE <br /> <br />Our current CAP/DA Progratg method of handling attendant/aide services will be modified to <br />allow our aged and disabled ~cipients or their representatives to assume the responsibility of <br />deciding who will meet their heeds, how their needs will be met, and the quality of care received. <br />The steps below will be takefi to assure the consumer is equipped to handle self-directed care: <br /> <br />1) <br /> <br />The consumer will be trailaed within 45 days of approval of the FL-2 (medical form required <br />for CAP/DA participatior0 to move from the traditional CAP/DA In-Home Aide Services <br />provided by a home care itgency to self-directing personal care services. The client will also <br />be informed of their comlilete autonomy in selecting an attendant/aide to provide care. <br /> <br /> <br />
