resources can be utilized to assure consumers are able to accomplish their daily activities of <br />living. <br /> <br />An introductory letter explaining the new concept and an initial survey will be mailed to current <br />recipients who have direct care workers through Home and Community Care Block Grant <br />Services or CAP/DA Services. Even though Personal Care Services can not be served under the <br />consumer-directed grant, we will include them in the survey because an increase in their care <br />needs may identify them as potentially appropriate for CAP/DA Services. We will also request <br />that the local Department of Aging distribute letters to persons participating in the Caregiver and <br />Respite Programs to determine how they see consumer-directed services as affecting their lives. <br />The survey will cover such points as: <br /> <br />· What services do you need to help you in the home? <br />· What assistive devices and equipment do you need to help you complete bathing, <br /> feeding, dressing, cooking, or transferring? <br />· Who will help you shop for needed purchases? <br />· Do you want the freedom to choose who will work for you? , <br />· Do you want the freedom to choose what type of services and supplies will best meet <br /> your needs? <br />· Will you want a representative to make decisions for you or help you make decisions? <br />· Do you feel you can train and supervise the people who will work for you? <br />· If the person you hired is not satisfactory, how will you handle the situation? <br />· Will you be able to form a backup plan when the direct care worker is unable to report for <br /> work or has to be replaced? <br />· Are you willing to ask for help and advice from our agency if you need it? <br /> <br />The agency intends to contact the consumer on a monthly basis to discuss the quality of care, <br />services and equipment received, as well as to gauge current satisfaction. The care advisor will <br />visit with the consumer on a quarterly basis to the quality of care, identify aTny changes in <br />condition that may necessitate a reassessment, and to assure that the needs identified on the plan <br />of care are being met. Also, a survey will be completed 10 months into the project to determine <br />how the clients feel about self-directing their care. <br /> <br />4. FUNDING SOURCE <br /> <br />Medicaid will be the primary funding source to deliver aide/attendant consumer-directed <br />services. As a CAP/DA lead agency we will be bound by the preset cost limits currently dictated <br />by the program, $2553 per month the clients requiring intermediate care and $3360 for clients <br />requiring skilled level of care. <br /> <br />6 <br /> <br /> <br />
