interviewing, hiring, supervising, evaluating and terminating attendant(s)/aide(s). The <br />orientation will also address wages, benefits, and the payment schedule. There will also be a <br />consumer handbook devised that will contain employer reference materials. The handbook <br />will be given at the orientation visit. <br /> <br />The attendant(s)/aide(s) will receive an orientation to the philosophy of consumer directed <br />service delivery. The attendant(s)/aide(s) will receive specific training on the <br />employer/employee relationship they will have with the consumer, the purpose of the service <br />plan agreed upon and completed by the consumer and attendant/aide, applicable employment <br />forms, timesheets, and payment schedule. It will also be explained to the attendant/aide that <br />it is required they demonstrate competence to perform the tasks assigned by the consumer in <br />a manner to assure health, safety and communication. <br /> <br />9. FIELD TEST EXISTING MATERIALS, SAMPLES PROVIDED OR NEW <br />MATERIALS <br /> <br />The current CAP/DA Assessment, along with a Self-Assessment to be developed, will be used to <br />identify individual needs. The CAP/DA Plan of Care will be used to track services provided to <br />the client and assure that services are provided within the cost limits established by DMA. The <br />current monitoring form in use will serve to readily identify problems, but still encourage <br />consumer direction. Other needed forms will include a consumer bill of rights, an agreement <br />which clearly delineates the roles of care advisor, consumer, and attendant/aide. Current forms <br />to be modified for consumer use include task sheets, in-home aide service plans, and time sheets. <br />We will also be glad to test any sample forms developed by the Consumer-Directed Services <br />Workgroup. <br /> <br />10. TIMELINE <br /> <br />A projected timeline of activities for the 14-month period of the CDS demonstration is attached. <br />DMA is pursuing a Medicaid waiver to incorporate consumer-directed services for people who <br />receive CAP/DA. Depending on when the Medicaid waiver is received, our timeline may be <br />affected. (See Attachment A) <br /> <br />11. FINANCIAL MANAGEMENT SERVICE CAPACITY <br /> <br />Financial Management services will be handled through a partnership with Good Health <br />Services, Inc. (GHS). GHS currently contracts with the Cabarrus County DSS In-Home Aide <br />program to provide certified and non-certified nursing assistants for the In-Home Aide program. <br />GHS is responsible for all human resources and payroll functions for the program. Under CDS, <br />GHS would act as the fiscal agent on behalf of the consumers. The consumer would make all <br />employment decisions and approve the attendant(s)/aide's timesheet. GHS would ensure that all <br />bookkeeping and payroll functions are completed. <br /> <br />l0 <br /> <br />2:7-5 <br /> <br /> <br />
