Collecting, maintaining, and sharing data, experiences and outcomes with the Real <br /> Choice grant proje4~t and relevant workgroup: We will develop a reporting form to <br /> share information wi~h the Real Choice grant, and would be willing to attend all <br /> workgroup meetings~t~ o share experiences and outcomes of our CDS program. <br /> d. Reporting on a qu~irterly basis about progress to date: We are developing a database <br /> of information that ~cludes outcomes and work measures that will be shared quarterly <br /> with the Real Choic~ grant. <br /> <br />14. SUSTAINING CONSUMER-DIRECTED SERVICE <br /> <br />It is our plan to make this p~gram as cost neutral as possible. This will be accomplished by <br />utilizing experienced staff td provide training to new staff. We feel that this program will be able <br />to be sustained, even in a period of funding reductions, due to our belief that a CAP care advisor <br />can manage more cases than~a CAP case manager. This will allow more clients to be served <br />even with staff reductions. <br /> <br />15. FISCAL MANAGEM~ NT <br /> <br />The CAP/DA Plan of Care v~i.~'ll assure that services are provided within the specified cost <br />constraints. Accounting prot~edures will be developed within the Cabarrus County DSS to track <br />how the grant funds are utilized. As a matter of routine, the Cabarrus County Finance <br />Department receives and disperses all monies received by county agencies. There will be a dual <br />system in place to assure acgOuntability for the disbursement of grant funds. <br /> <br />13 <br /> <br /> <br />
