DOA-734 <br />(revised 1/01) <br /> <br /> July 1, 2003 through June 30, 2004 <br />Home and ~ommunity Care Block Grant for Older Adults <br /> <br /> ~ Community Service Provider <br /> <br /> Standard Assurances <br /> <br /> Cabarrus County Department ~ Social Services agrees to provide services through the Home and <br /> (Name of Provider) <br />Community Care Block Grant, als specified on the Provider Services Summa _ry (DOA-732) in <br />accordance with the following: <br /> <br />o <br /> <br />Services shall be provid~ in accordance with requirements set forth in: <br /> <br />a) <br />b) <br /> <br />c) <br /> <br /> The County Fun~ling Plan; <br /> The Division of &ging Home and Community Care Block Grant Procedures Manual for <br /> Community Ser6ice Providers; and <br /> The Division of&ging Services Standards Manual, Volumes I through IV. <br /> <br />Community service proqiders shall monitoi' any contracts with providers of Block Grant <br />services and take approl~'iate measures to ensure that services are provided in accordance with <br />the aforementioned doc .t~xnents. <br /> <br />Priority shall be given ta providing services to those older persons with the greatest economic <br />or social needs, with particular attention to low-income minority individuals. The service needs <br />of Iow-income minority i~lderly and rural elderly will be addressed in the mariner specified on <br />the Methodology to Ad,ess Service Needs of Low-Income Minority Elderly and Rural Elderly <br />format, (DOA-733). <br /> <br /> ,, <br />The following service a~thorization activities will be carried out in conjunction with all services <br />provided through the B14ck Grant: <br /> <br />a) <br />b) <br />c) <br />d) <br />e) <br /> <br />Eligibility detemgi. 'nation; <br />Client intake/registration; <br />Client assessmedt/reassessments and quarterly visits, as appropriate; <br />Determining the [tmount of services to be received by the client; and <br />Reviewing cost sharing/voluntary contributions policies with eligible clients. <br /> <br />All licenses, permits, bo~ds, and insurance necessary for carrying out Block Grant Services will <br />be maintained by the co~nunity service provider and any contracted providers. <br /> <br /> <br />
