STATEMENT OF WORK NARRATIVE <br /> <br />This portion of the proposal should give reviewers a clear picture of the proposed services and the <br />capability of the offeror to deliver the proposed services. Proposers should follow the alphabetical and <br />numerical sequence of the format described below. If a section does not apply, write "N/A" or "Does not <br />apply" by the respective section. <br /> <br />A. PROGRAM MANAGEMENT PLAN <br /> <br />Provide a brief background/history of the organization. Describe the mission of your organization. <br /> Include a program organizational chart that outlines the administration of the organization for the <br /> proposed project. Cabarrus County Department of Social Services is the county <br /> department entrusted with the responsibility of providing for the needs for adults, <br /> children, and families to ensure that basic necessities are met and populations who are <br /> susceptible, i.e~ children and elderly/disabled adults are protected from abuse or neglect. <br /> It is also mandated to provide crisis and employment services to able-bodied adults to <br /> enable them to better provide for their families. These services are reflected in the <br /> Mission of. Cabarrus DSS, which states that "the mission of the Cabarrus County <br /> Department of Social Services is to enhance the quality of life in our community through <br /> programs which help the vulnerable, the aged, the sick, the poor, and the children, helping <br /> our citizens to become self-supporting; to be self-sufficient; to prevent or correct abuse, <br /> neglect, and exploitation; to avoid institutional care when possible; and, to secure <br /> institutional care when appropriate. (an organizational chart is attached outlining the <br /> administration of the PREVENTS program) <br /> <br />Identify lines of authority and supervision for program operation. The line of authority for the <br />PREVENTS program is as follows: <br /> · James F. Cook, Jr. Director of Social Services <br /> · James H. Polk, Program Administrator of Adult and Family Services <br /> · Monica Green, Supervisor of Employment Services <br /> · PREVENTS worker <br /> <br />Identify all staff positions necessary for the operation of this program in the JobLink Career <br />Center. One full-time staff position would be necessary to operate the PREVENTS <br />program. This would be the PREVENTS Worker. Complete job description form for each of <br />the WIA-funded positions. Include staff job duties, education, and experience. A ,job <br />description is included using the prescribed form. <br /> <br />Specify the JobLink facility where services will be delivered and the amount of space available. <br />Discuss the facility's capabi/ities of serving the handicapped. Much of the services for the <br />customers served will take place in the Cabarrus JobLink Career Center and Cabarrus <br />Cou. nty DSS. There is adequate cubicle space in both locations and interviewing area for <br />the PREVENTS worker to counsel and work with the customers. The facilities can <br />accommodate handicapped customers, as both are on one floor, with no steps, and has <br />adequate entrance and exit with wide walkways. <br /> <br />Describe your agency's role as a partner agency of the JobLink Career Center. Provide copy of <br />formalized agreement. Cabarrus County Department of Social Services serves as a core <br />partner with the Cabarrus JobLink Career Center. A departmental representative sits on <br />the JobLink Management Team. <br /> <br />B. CORE SERVICES <br /> <br />Describe the activities and methods that will be used for outreach and recruitment for potential <br /> <br />WIA Title I Adult/Dislocated Worker RFP <br />April 2002 <br /> <br /> <br />
