will train the contractor to complete and correctly enter the required eligibility data into the WIA MIS <br />WorkforcePlus system. The Contractor shall have three (3) key operators: WorkforcePlus Intake <br />Specialist, the WorkforcePlus Certification Specialist and the Workforce Plus Case Manager and <br />Case Administration Manager. Upon approval by the Centralina Local Area Administrator, the <br />contractor may be approved for use of the system by one (1) or two (2) contractor staff. The WIA MIS <br />WorkfomePlus system requires three (3) individuals to maintain and enter data into the system. All <br />data collected and entered will be stored in a central database by Softscape, Inc. <br /> <br />The WorkforcePlus Intake Specialist will prepare and collect all verification documents and <br />enter eligibility data into the WIA Workforce Plus system and will initiate the applicant's ISS. After <br />eligibility, the WorkforcePlus Intake Specialist will then forward the collected data to the <br />Workforce Certification Specialist for approval. <br /> <br />The Workforce Certification Specialist will review the'collected data and verification <br />documents to verify eligibility and in turn, submit approval to the WorkforcePlus Intake · <br />Specialist who will then submit the eligible applicant to the WorkforcePlus Case Manager <br />and Case Administration Manager. <br /> <br />The WorkforcePlus Case Manager and Case Administration Manager will determine and <br />place the applicant in the appropriate adult services and shall be required to maintain case <br />management notes on all applicants. <br /> <br />4. All Workforce Plus administrators shall be required to maintain a degree of case <br /> management notes as provided on the WorkforcePlus system. <br /> <br />5. All Workforce Plus administrators are required to maintain a folder with signed and printed <br /> documents of all applicants and each applicant's ISS. <br /> <br />All Workforce Plus administrators will maintain and document on the ISS a record of referrals <br />made to other service providers and other Human Resources Agencies for supportive ' <br />.services. <br /> <br /> Follow-up Services 663.150(b) <br /> <br />WIA service providers are required to provide folloW-Up services for Adults and Dislocated Workers, <br />for a minimum of 12 months following the first day of employment, to registered participants who are <br />placed in unsubsidized employment. <br /> <br />Indicate the staff person(s) responsible for follow-up services. <br /> <br />To be hired PREVENTS Worker Not assigned <br />Staff -Assigned-- JobTitle' Phone #- <br /> <br />Staff Assigned Job Title Phone # <br />Staff Assigned Job Title Phone # <br /> <br /> Coordination/Linkages <br /> <br />WIA service providers will be required to coordinate services with JobLink Career Centers and other <br />local agencies.
