Briefly describe what the program expects to achieve and why. The narrative should address each of the <br />items (1) Identify the staff and their responsibilities to the ]~rogram, (2) describe the physical facilities in <br />~h the services will be delivered, (3) discuss any areas of concern that could affect the success of the <br />program (4) discuss why these resources used in the manner described should be expected to alter <br />inappropriate behm, ior, (5) describe the referral, screening and admission procedures, (6) describe the <br />means of working with youth and families, (7) describe the daily program activities and, (8) describe the <br />termination procedures and (9) interaction with juvenile court. <br />1. Program Staff Include: Chat:les ltill, Cabarrus Center Director supervises <br />all aspects of center sera-ices; Vince Pastore; Ph.D., C&Y Coordinator pro- <br />s-ides day-to-day supervision to the entire Staff involved in the OJJ program; <br />Rick Flelms, MA provides the on-site sen-ice delivery to the OJJ s-taft, clients <br />and families, coordinates referrals and consults to the court; Betsy Karas <br />pro~Sdes MH treatment for referred clients; Stephanie Pierce, Pam Burton, <br />Kelly Grey, Kelly Spain and Rob ltilliard pros.-ide SA sera'ices to clients; <br /> Philip Nofal, MD provides medication consultation, assessment and monitoring <br /> of medication for referred clients; Swati Nirodi,Christie Heckler,Leann Kimpto <br /> n pro,.Sale Mental Health Services to referred clients; Revella Nesbit and <br /> Tonya Brown provide Family Preservation set, Sees to referred clients. <br /> <br />2. Physical Facililties: Cabarrus County Court House is the site of direct <br />service provSsion for the primary grant position held by SA therapist. <br />Piedmont Behavioral l~Iealthcare, Cabarrus Center, is the sight of 'all <br />other service deliovery cited in #1 of this narrative except for Family <br />Preser~,ation which is supplied in the family home. <br /> <br />3. Areas of Concern: Youth referred to ser~qce generally have a long history <br />:6f acting out, family dysfu,nction, and substance abuse. <br /> Because of this it is important that these clients and their families receive <br /> treatment that gives them the opportunity to succeed. This entails family <br />lintervention and coordination. FailUre of the family and community to be <br />involved in the treatment will decrease the likelihood of success. <br /> <br /> 4. Funding for the half time SA position based at the court house <br /> insures timely access to the clinician for screenings, assessments, testing <br /> tnd consultation. This facilitates good communication, timely decision <br /> making and increased therapeutic efficacy. Referrals for additional <br /> SA services insures that the family will have access to other <br /> needed sera'ices such as therapy, groups, and psychiatric sen'ices. This <br /> continuim of SAflV~ ser~'ices increase the likelihood of treating <br /> family and child dysunction effectively. <br /> <br />5. A. Funding for clients sersSces will be covered through the DJJ grant. <br /> B. Referrals can be made through SA therapist on-site at the court house <br />or through the Cabarrus Access team at 939-1100 for other sera'ice <br />SA Ther. <br />will conduct screenings for all direct referrals and coordinate additional <br />treatments efforts by Cabarrus Center for the client. <br /> C. Terminations'will be based on completion of set'Sees, noncompliance <br />with the treatment plan, or referral to other sersSccs outside the Cabarrus <br />Center. This will be coordinated with the clinician and court counselor. <br /> <br /> <br />
