STATE OF NORTH CAROLINA <br /> <br />CONTRACT FOR SERVICES <br /> <br />COUNTY OF CABARKUS <br /> <br />This contract for services made and entered into this ~ day of May 2001 by and <br />between Cabarrus County (hereinafter referred to as "County") and Carolina Addiction <br />and Reco. very Environment, Inc. (hereinafter referred to as "Provider"): <br /> <br />WlTNESSETH: <br /> <br />WHEREAS, the County has chosen to operate a community based correction program <br />and wishes to provide intensive and regular outpatient substance abuse treatment ser¥ices <br />to offenders sentenced to that program through District and Superior Court and for the <br />exclusive use of the Division of Community Corrections; and, <br /> <br />WHEREAS, the Provider agrees to provide the services as described herein according to <br />the terms and conditions set forth herein; and <br /> <br />WHEREAS, the County is the Grantee for the NC Deparmtent of Corrections-Criminal <br />Justice Partnership Program Grant Funds which provide in part for the purchase of said <br />substance abuse treatment services; <br /> <br />THEREFORE, THE County and Provider do hereby mutually agree as follows: <br /> <br />SECTION I: SCOPE OF SERVICES <br /> <br />The Provider shall perform the following services as included in Addendum A (Proposal <br />for Services) submitted by CARE, for those offenders sentenced to the Cabarrus County <br />Drag Treatment Program and deemed appropriate for the services: <br /> <br />A. Individualized Assessments: The Provider.shall provide within 48 hours after <br /> referral, each offender an individualized substance abuse assessment which <br /> consists of administration of a standardized written assessment instrument and <br /> a face-to-face interview conducted by a qualified substance abuse counselor. <br /> <br />'B. Intensive Outpatient Treatment (IOPT): The Provider shall provide each <br /> offender assessed to alcohol/drug dependence a ten-month (10) Multi-phase <br /> IOPT consisting ora minimum of 148 treatment hours distributed as follows: <br /> <br />Phase One - Intensive Treatment: Eight weeks, three group sessions per <br />week, three hours per session, plus a minimum of one individual session bi- <br /> <br />Page 1 of 9 ~--/~ <br /> <br /> <br />
