A. That it acknowledges that in receiving, storing, processing or otherwise using or <br /> dealing with any such information, the Provider and its employees are fully bound <br /> by the provisions of North Carolina General Statutes 122-8.1 as amended and all <br /> other Federal and State Laws and Regulations that govern and guarantee the <br /> treatment rights and confidentiality of individuals receiving substance abuse <br /> treatment services. <br />B. To resist any effort to obtain access to client treatment information not expressly <br /> provided for in the aforementioned paragraph. <br />C. Provider shall obtain appropriately signed forms allowing for the release of <br /> information that may be necessary for the Provider to conform to the requirements <br /> of the NC State Criminal Justice Partnership Program. <br /> <br />SECTION XII: EQUIPMENT COSTS AND OFFICE SPACE <br /> <br />The Provider shall supply, at its own expense, all equipment, tools, .materials and/or <br />supplies required to deliver treatment services contracted hereunder. The Provider shall <br />provide office space for necessary staffmembers. <br /> <br />SECTION XIII: CLIENT ADMISSION <br /> <br />In accordance with the provisions of Title IV of the Civil Rights Act of 1963, and the <br />regulations of the United States Department of Health and Human Services, issued <br />· pursuant thereto, the Provider agrees to extend all funds allocated in such a manner that <br />no person will be excluded from participation in, be denied the benefits of, or otherwise <br />be subject to discrimination under purchased services on the grounds o~' race, religion, <br />color, national origin or sex. <br /> <br />Page 8 of 9 <br /> <br /> <br />
