Briefly describe what the program expects to achieve and why, The narrative should address each of the items <br />(1) Identify the staff and their responsibilities to the program, (2) describe the physical facilities in which the <br />services will be delivered. (3) discuss any areas of concern that could affect the success of the program, (4) <br />discuss why these resources used in the manner described should be expected to alter inappropriate behavior. <br />(5) describe the referral, screening and admission procedures, (6) describe the means of working with youth <br />and families, (7) describe the daily program activities. (8) describe the termination procedures and (9) <br />interaction with juvenile court. <br /> <br />A full time nurse is assigned to Kannapolis Middle School. She is supervised by a <br />school nursing supervisor from the Cabarrus Health Alliance. The nurse will <br />collaborate with school personnel (principal, guidance counselors, teachers, social <br />workers), the court counselor and other community agencies to target high-risk youths <br />who have been identified as "at-risk" adolescents. Through one-to-one interaction with <br />the student on campus on a daily basis and on visits with the family, the nurse will <br />work with the teen to reduce high-risk behaviors, encourage him/her to and remain in <br />school and out of the court systems. An individualized intervention plan will be <br />established for each student served by this program. The student, parent(s), principal <br />or designee, nurse and court counselor will sign the intervention plan. A parent, <br />student, and staff conference will be held to initiate the plan and periodic monitoring <br />will be implemented to assess the student's progress and adjust the plan as needed. <br /> <br />Referrals to the nurse may come from the school staff as well as agencies outside of the <br />school. The nurse will serve as the 'clearinghouse' for the "at-risk" student and be <br />his/her daily "in school" support person. The nurse will use her knowledge of <br />community agencies and resources to assist the youth and his family with an array of <br />identified needs. When a referral is made to the nurse, she will follow-up with the <br />youth and/or the family until resolution to the referral is complete. <br /> <br /> <br />
