SAIL also has the resources easily available to fully evaluate each individual <br />as necessary for a dual diagnosis. <br /> <br />WORK PLAN <br /> <br />SCREENING AND CLINICAL EVALUATION <br /> <br /> The client's relationship with SAIl. will begin with the substance abuse screening. <br />The client will call for an appointment to be made available within 48 hours of initial <br />contact. This screening will be conducted in an interview format with a substance abuse <br />professional. The screening will contain questions about the client's substance abuse <br />history including, but not limited to: Chronic and severity of use; Symptoms of chemical <br />dependency/abuse; Effect of chemical use on vocational, legal, financial, familial, <br />emotional, educational, mental, and physiological areas of the client's life. The client <br />will also be given a Substance Abuse Subtle Screening Inventory (SASSI) III test, which <br />will be used as an assessment tool. Additional testing may be required in certain <br />circumstances. Once the information has been gathered and the testing has been <br />evaluated, the screening will be reviewed by the staff'ofthe program. Recommendations <br />will be based on DSM IV and ASAM criteria. The results of the screening and <br />recommendations will be reviewed with the client and case manager of the program. Ifa <br />client presents with a dual diagnosis or evidence ora mental health condition; the client <br />will be evaluated further to allow for appropriate care or referral. If the client presents <br />with a coexisting condition that can not be addressed by the SAIL program, a referral to <br />an appropriate community resource will occur. The client may also request a more <br />intense level of care (inpatient treatment), but will not be permitted to enter a level of <br />care below the recommendation. SAIL's approach to treatment is team oriented. Thus, all <br />decisions regarding treatment will be shared during treatment team meetings. The <br />treatment program begins at the time of the assessment. <br /> <br />Bio-Psychological Assessment <br /> <br /> During the bio-psychological assessment, the clients will be questioned about <br />many areas of their lives including, but not limited to, substance abuse history, family <br />history, vocational, educational, legal, medical, psychological, leisure, and financial <br />history, and current mental status and functioning. The counselor assigned to the client <br />will conduct this interview. The bio-psychological assessment will be utilized to <br />accomplish multiple tasks. It allows for the identification of issues that will be addressed <br />during the treatment process. The assessment also identifies family needs that will be <br />addressed during multifamily therapy. Finally, it will be used to identify any coexisting <br />conditions. A referral will then be made to an appropriate agency. Once the bio- <br />psychological assessment has been completed, an individualized treatment plan will be <br />developed based on the client's needs, issues, strengths, and weaknesses. <br /> <br /> <br />
