Page 2 <br /> <br /> SlX MONTHS MEASURABLE OBJECTIVES STATUS <br /> <br /> (Continuation Programs Only) <br /> <br />In the space provided, please list each measurable objective in your current program <br />agreement and indicate the degree to which your program has been successful in achieving <br />them. (Use data from first six months of the fiscal year.) <br /> <br />1) 100% of clients will improve school attendance (to every day) as documented <br /> by school notes and regular communication with school officials. <br /> <br /> We have had 1 Cabarrus County boy enrolled in our program during the first six months <br />of this fiscal year. We anticipate 1 boy from Cabarrus County entering our program for the <br />remainder of this fiscal year. <br /> <br />2) 75% of clients will improve school performance from failing grades to at least a <br /> C average, as evidenced by daily school notes and report cards. <br /> <br /> We have had 1 Cabarrus County boy enrolled in our program during the first six months <br />of this fiscal year. We anticipate I boy from Cabarrus County entedng our program for the <br />remainder of this fiscal year. <br /> <br />3) Delinquent and/or inappropriate behaviors will decrease by 80% as evidenced <br /> by no new court referrals, positive home visits, and daily progress notes by Resident <br /> Counselors. <br /> <br /> We have had 1 Cabarrus County boy enrolled in our program during the first six months <br />of this fiscal year. We anticipate 1 boy from Cabarrus County entering our program for the <br />remainder of this fiscal year. <br /> <br />4) Social and communication skills wilt improve by 50% through use of role play, <br /> counseling, family meetings, outside activities, and positive reinforcement from <br /> Resident Counselors. Evidenced by positive home notes, school notes, and <br /> interpersonal relations at the group home. <br /> <br /> We have had 1 Cabarrus County boy enrolled in our program during the first six months <br />of this fiscal year. We anticipate 1 boy from Cabarrus County entering our program for the <br />remainder of this fiscal year. <br /> <br /> <br />
