PART III (of 3 Parts) <br /> <br />A.) Weatherization Assistance Program. The following shall apply to the Weatherization <br />Assistance Program as noted in Part I, Section 4A: <br /> <br /> Section 1: Program Guidance. The Subgrantee shall provide to the Subcontractor <br />copies of all rules and regulations, instructions, and other documents pertaining to the <br />Program. In no instance shall the Subcontractor take action on any rule, regulation, or <br />instructive printed in the Federal Register or other document, without first notifying the <br />Subgrantee of such intention. In the event that rules, regulations, or procedures are revised <br />or amended, the Subgrantee will forward such revisions or amendments to the <br />Subcontractor and thereby make them a part of this Agreement. <br /> <br /> Section 2: Certification of Applicants. The Subcontractor shall give priority to <br />identifying and providing assistance under this Program to elderly and handicapped <br />Iow-income persons and to provide assistance to those persons whose income is at or below <br />150% of the poverty level as determined by the Office of Management and Budget, or who <br />received cash assistance payments under Title IV or XVI of the Social Security Act or other <br />applicable State or local law. Applicants must have received assistance during the <br />preceding 12 months in which weatherization funds are being appropriated. The <br />Subcontractor must certify the eligibility of each household by completing the DOE <br />Homeowner Certification Form, EIAo29D. Eligibility shall be certified no less than six (6) <br />months prior to receipt of weatherizatio,~ assistance. In those instances where dwellings are <br />not owned by the applicant, the Subcontractor shall execute an Agreement for Participation <br />for Leased Dwellings. The Subcontractor must also obtain a Release Work Order from the <br />applicant prior to the actual weatherization activities. <br /> <br /> Section 3: Prioritization of Client Categories. The Subcontractor shall give <br />priority to identifying and providing assistance under this Program to elderly and <br />handicapped Iow-income persons. The Grantee has established statewide completion <br />goals of 39% elderly, 14% handicapped and 47% other. In addition, the Grantee has <br />established a statewide completion goal of 31% rental units. The Subcontractor shall, <br />to the maximum extent practicable, attempt to meet these percentage goals. <br /> <br /> Section 4: The Subcontractor shall not exceed an average cost of $2,032 per <br />dwelling unit completed (not including administrative costs) nor shall it exceed $1,250 <br />materials cost on any individual unit completed. Where it is determined that the <br />average unit cost or the individual unit materials cost is insufficient to meet client <br />needs, the Subcontractor shall submit a written request for a Materials Cost Waiver to <br />the Subgrantee who will in return request a waiver from the Grantee for approval. <br /> <br /> Section 5: Schedule of Implementation. The Subcontractor agrees that a minimum <br />of 45 dwelling units are to be weatherized during the grant period defined in Part I, <br />Section 3. In addition, the Subcontractor agrees that if the rate of completion of weathedzed <br />units fails to a level that will prevent the Subcontractor from utilizing the full amount of funds <br />provided under this contract, then the Subcontractor will notify the Subgrantee of such a <br />situation and allow the renegotiation of the contract to a funding level consistent with the <br />Subcontractor's current rate of production.
