He will ask the client what their needs are and explain what can or <br /> can not be repaired under the Urgent Repair Program. <br />2. Work write-ups <br /> Program Manager/Weatherization Coordinator will complete a work write-up <br /> of all repairs/modifications. <br />3. Contractor Procurement <br /> Some work shall be performed by recipient agency as the general contractor <br /> where this method is used. The Weatherization Coordinator shall complete a <br /> cost estimate of all materials and labor to complete the work. he shall to the <br /> maximum extent use the proper procurement process to purchase materials <br /> and supplies. Some work may be done by recipient agency as the General <br /> Contractor and part of the work sub-contracted by licensed sub-contractors. <br />4. Work performed by other contractors when work is contracted to a general <br /> contractor. Weatherization Coordinator will complete a work write-up to spell <br /> out materials to be used and the work process. These work write-ups and bid <br /> invitations shall include the clients' name, address and phone number. The <br /> Weatherizafion Coordinator shall solicit telephone bids from contractors. <br /> Client must allow these contractors access to their homes so the contractor <br /> may inspect and make measurements to bid on the work. Contractors shall <br /> carry proper insurance. Homeowners must carry homeowners insurance. <br />5. Contracts <br /> Contract forms shall be completed by Weatherization Coordinator for each <br /> client's home whether work is done in house by recipient as the general <br /> contractor or work is performed by an outside general contractor. <br />6. Change Orders <br /> All change orders must be in writing and signed by the Agency contractor and <br /> home owner, prior to any work being performed that was not spelled out in the <br /> original contract. <br />7. Pre Construction Conference <br /> Weatherization Coordinator shall set a date, time and location to meet with the <br /> home owner, general contractor, to go over work write-up, specifications and <br /> contract. A pre-construction conference record sheet shall be completed and <br /> signed and dated by representatives of the agency, contractor and home <br /> owner. They shall agree on a date for work to begin. The contract shall be <br /> signed at this time by all parties involved. <br />8, Grant Agreement <br /> Home owner must sign a completed grant agreement before funds can be <br /> used on their home. This grant agreement must be witnessed by a properly <br /> authorized individual. <br />9. Inspections <br /> Agency shall from time to time inspect work as it is being performed to insure <br /> that work is being properly performed. <br /> <br />A final inspection shall be performed after all work has been completed to <br />insure that all materials have been installed, and that work has been <br />completed in a workman like manner. Contractor will not be paid until work <br />has been satisfactorily completed. After final inspection home owner will <br />need to sign an "Owner Certificate of Satisfaction" form. Weatherization <br />Coordinator will insure that this form along with a "Contractor's Release of <br />Liens" form is complete and signed. <br /> <br /> <br />
