Home and Community Care Block Grant for Older Adults <br /> <br /> County Funding Plan <br /> <br />DOA-733 <br />(Rev. 1/98) <br /> <br />METHODOLOGY TO ADDRESS SERVICE NEEDS OF LOW INCOME-MINORITY ELDERLY <br /> (Older Americans Act, Section 306(a) (5) (A) (ii) <br /> <br />Community Service Provider Cabarrus County Department of Aging <br /> <br />County Cabarrus <br /> <br />July 1, 1998 through June 30, 1999 <br /> <br />The Older Americans Act requires that the service provider attempt to provide <br />services to low-income minority individuals in accordance to their need for aging <br />services. The community service provider shall specify how the service needs of <br />low-income minority elderly will be met through the services identified on the <br />Provider Services Summary (DOA-732). This narrative shall address outreach and <br />service delivery methodologies that will ensure that this target population is <br />adequately served and conform with specific objectives established by the Area <br />Agency on Aging, for providing services to low income minority individuals. <br /> <br />GOAL: To ensure the participation in services provided through the Cabarrus <br />County Department of Aging of the low income minority elderly in the community. <br /> <br />The Cabarrus County Department of Aging provides services to all older adult <br />residents regardless of race or economic status. Brochures are distributed <br />throughout the community that promote the services provided by our agency. <br />Services are also promoted through local churches and through speaking <br />presentations to senior citizen and local civic groups. <br /> <br />Our current client base includes a substantial number of minority clients and four <br />of our congregate nutrition sites are located in minority and rural areas. We will <br />continue to distribute information in predon~inately minority and/or low income <br />neighborhoods, promote our services through local churches, make personal <br />presentations to groups, and place informational brochures in local doctor offices <br />and hospital. <br /> <br />Our local newspaper, The Independent Tribune, prints and distributes our Sunset <br />Outlook newsletter bi-monthly to 9.7,000 subscribers. This allows publicity of our <br />programs and services to reach the entire county. We also coordinate service <br />referrals with other human service agencies and receive referrals for services from <br />our local Home Health Agencies and Dept. of Social Services. <br /> <br /> <br />
