DOA-733 <br />(Rev. 1/98) <br /> <br />Home and Community Care Block Grant for Older Adults <br /> <br />County Funding Plan <br /> <br />Methodology to Address Service Needs of Low-Income Minority Elderly <br /> (Older Americans Act, Section 306(a)(5)(A)(ii) <br /> <br />Community service Provider Cabarrus'County Department of Social Services <br /> <br />County Cabarrus <br /> <br />July 1, 1998 through June 30, 1999 <br /> <br />The Older Americans Act required that the service provider attempt to provide services to low- <br />income minority individuals in accordance to their need for aging services. The community <br />service provider shall specify how the service needs of low-income minority elderly will be met <br />through the services identified on the Provider Services Summary (DOA-732). The narrative <br />shall address outreach and service delivery methodologies that will ensure that this target <br />population is adequately served and conform with specific objectives established by the Area <br />Agency on Aging, for providing services to iow-income minority individuals. Additional pages <br />may be used as necessary. <br /> <br />GOAL: To ensure participation in services and programs by low income minority elderly <br />persons at a minimum rate of the percentage of low income minority elderly in the community. <br /> <br /> Cabarrus County Department of Social Services provides assistance to all individuals <br />requesting services regardless of race with most of our programs serving low income individuals. <br />The type or extent of services clients receive are based solely on clients' need and availability of <br />funding. <br /> <br /> Through the In-Home Aide Program, 48% of the clients served are in the minority <br />population. This exceeds the 13.8% minority population of Cabarrus County which is reflected <br />in the 1990 census statistics. We will continue to strive to serve the low income minority <br />population. In our Community Alternatives Program (CAP), 33% of the clients served are in this <br />group. <br /> <br /> Outreach by the agency has been accomplished by newspaper articles, brochures, home <br />visits by social work staff, and contacts with other agencies. Information was distributed at a <br />Health Fair held in the Logan area in March, 1997. Since funding is limited for these programs, <br />we will not implement any new outreach programs; however, we will continue with our current <br />outreach efforts which have proven effective in the past. Based on current need for services, we <br />only have sufficient funds to maintain current clients. We continue to receive requests from <br />clients that meet the need for In-Home Aide Services. We will assess these requests if funding <br />becomes available, so they can be added to our program. <br /> <br /> As an agency that receives funding from several sources, we will assess each case to be <br />sure that funding is used to best meet the needs of the clients and that Older Americans Act funds <br />are targeted towards low income minority individuals. <br /> <br /> <br />
