ADULT DAY SERVICES CONTRACT <br /> <br />Cabarrus County Department of Aging <br /> L.I.F.E. Center, Inc. <br /> <br />CABARRUS COUNTY <br />CONCORD, NORTH CAROLINA <br /> <br />This contract is entered into this first day of July, 1998, by the L.I.F.E. Center, Inc., hereinafter <br />called "the Contractor" and Cabarrus County, as represented by the Cabarrus County Department <br />of Aging, hereinafter called "the County." <br /> <br />The Contractor and the County, in consideration of the mutual covenants, promises and agrees <br />herein contained, agree as follows: <br /> <br />The Contractor shah provide to the County the services set forth in the Contract documents <br />consisting of providing Adult Day Services for the County for a one year period from July 1, 1998 <br />through June 30, 1999. <br /> <br />I. RESPONSIBILITY OF COUNTY: <br /> <br />The Cabarrus County Department of Aging shall be the "Community Service Provider" as defined <br />in the North Carolina Home and Community Care Block Grant Procedures Manual. (Effective <br />Date: July 1, 1992 Revised: February 17,-1997). ' <br /> <br />The Cabarrus County Department of Aging shall: <br /> <br />assume full responsibility for determining client eligibility and priority status for placement <br />utilizing HCCBG funds. (The County and the Contractor will work together in a <br />collaborative effort to collect data and process all appropriate screening / intake <br />instruments and necessary registration information with the County assuming the lead role <br />when appropriate.) However, it is agreed that the Contractor will have the final authority <br />to accept or deny services to any applicant, even if approved for HCCBG funded services, <br />based on physical, social and / or emotional characteristics that may exist. <br /> <br />conduct in home assessments when appropriate to collect and / or confimi information, in <br />collaboration with the Contractor, with the County assuming the lead role when <br />appropriate. <br /> <br />review "cost sharing" policies with the client in an effort to encourage voluntary <br />contributions from the client / caregiving family, in collaboration with the Contractor, with <br />the County assuming the lead role when appropriate. <br /> <br />provide the local match as required by the Home and Community Care Block Grant <br />(HCCBG) for Adult Day Services in Cabarms County, not to exceed $13,103.00. <br /> <br /> <br />
