Adult Day Services Contract <br />Page 2 of 4 <br /> <br />complete and process all appropriate reports to the Area Agency on Aging (AAA) and / or <br />the North Carolina Division of Aging (DOA). <br /> <br />6. attend AAA and / or DOA meetings and training sessions. <br /> <br />conduct audit / assessment visits on the Contractor as necessary to insure compliance with <br />HCCBG standards and collect necessary data to provide accurate documentation to the <br />AAA during the annual County assessment. (This includes on-site monitoring visits as <br />necessary.) <br /> <br />II. RESPONSIBILITY OF CONTRACTOR: <br /> <br />The Contractor shall be the "direct service provider" for all eligible Cabarms County residents <br />that receive Adult Day Services through HCCBG funding. <br /> <br />The Contractor shall: <br /> <br />make available to the County and / or AAA any / all records relating to clients that receive <br />Adult Day Services from the Contractor through HCCBG funding. <br /> <br />provide the County with a copy of a Certificate of Insurance that will verify adequate <br />current insurance coverage. Furthermore, the Contractor agrees to maintain in effect full <br />insurance coverage, in ameunts no less than the industry standard. <br /> <br />maintain appropriate Certification by the Division of Social Services to operate an Adult <br />Day Services program for each facility. (See attached, Exhibit A and Exhibit B · copies of <br />current certification). <br /> <br />notify the County immediately (same day) in the event of any adverse action regarding <br />Division of Social Services Certification that may place the Adult Day Services program in <br />a provisional, revocation or terminated status of operation. The Contractor will continue <br />to keep the County updated and informed concerning corrective action utilized to address <br />deficiencies with complete corrections made within the specified time frame to Division of <br />Social Services standards. <br /> <br />secure at no expense to the County, all required personnel and provide appropriate <br />professional training to meet all licensing requirements necessary to operate this Adult <br />Day Services program. <br /> <br />operate the Adult Day Services program at (1) Central United Methodist Church and (2) <br />Covenant Presbyterian Church, five days per week, Monday through Friday, except for <br /> <br /> <br />
