Adult Day Services Contract <br />Page 3 of 4 <br /> <br />designated holidays and inclement weather conditions, in which most cases the Cabarrus <br />County Schools policy will be followed. Furthermore, one day per month, the Contractor <br />will close the facilities for staff development. The Contractor will notify the County of <br />such closings within the first week of each month via their monthly calendar. <br /> <br />assist the County with the final assessment and development of the personal care plan of <br />each client. <br /> <br />following the screening / intake process and an orientation visit, if enrollment into this <br />program is approved, a schedule will be established and the contractor will be eligible for <br />reimbursement beginning with the first full day of attendance. <br /> <br />provide these Adult Day Services in accordance with North Carolina Division of Aging - <br />Adult Day Care Service Standards. <br /> <br />10. <br /> <br />collect all cost-sharing contributions from the clients and process these funds in <br />accordance with HCCBG policy. <br /> <br />II1. COMPENSATION AND METHOD OF PAYMENT: <br /> <br />The Contractor shall submit an invoice to the County no later than th6 third working day of each <br />month for the services provided in the previous month:' This time schedule is established in order <br />for the County to develop and process the monthly report to the AAA on or before the eighth of <br />each month in order to receive reimbursement for.these services from HCCBG funds: ? :- .-:-:-. :: <br /> <br />Each invoice will include an attendance / absentee report, specify transportation services provided <br />and a cost sharing revenue report for each client served through HCCBG funding during the <br />service period. <br /> <br />The County agrees to reimburse the Contractor at the rate of $23.00 for each unit of service (each <br />one day unit of service requires a minimum of six hours of care under HCCBG Service Standards) <br />successfully provided to eligible clients or for scheduled units of care, as opposed to the actual <br />units of service provided until the client is absent for ten consecutive scheduled days. <br />(Transportation provided by other means, i.e., family / friends, E&DTAP funds, etc., will not be <br />eligible for reimbursement.) <br /> <br />The County and the Contractor will closely monitor the attendance for each of the authorized <br />client placements, in an effort to efficiently utilize all reservation days assigned to this program. <br />While some clients will have a consistent regular attendance pattern, others may need to be <br />checked on daily with reminder telephone calls. Planned attendance will be confirmed and / or <br />substitute client replacements will be made with other pre-qualified participants who have a need <br />for Adult Day Services. The Contractor will notify the County each time a client is absent three <br />consecutive scheduled days. <br /> <br /> <br />
