Employment Transportation Assistance, Page 3 <br /> <br />Data Information Needs (Report): <br /> <br />The reporting form will be provided with the disbursement of funds to the county. <br />Counties must ensure that each local sub-recipient of the Work First <br />Transitional/Employment Transportation Assistance funds compile data for the project's <br />duration. Examples of specific data that must be kept for services provided by or contracted <br />through Community Transportation Systems (CT System) are the number of one-way <br />passenger trips, service hours and service miles provided. Other information to be reported <br />is the number of transitional Work First recipients receiving assistance from this program, <br />the number of total non-duplicated individuals served (transitional Work First and general <br />employment transportation), total allocation received, total expenses incurred, and total <br />interest earned from the funds (if applicable). In addition and all financial documentation <br />must also be kept by the county and each sub-recipient in accordance with state law. <br /> <br />Each county must provide written annual reports on the following information: <br /> <br />1. Number of one-way passengers transported; <br />2. Number of total non-duplicated individuals served; <br />3. Number of service hours provided; <br />4. Number of service miles provided; <br />5. Total expenses; <br />6. Total number of transitional Work First participants served (also include within #2); <br />7. Other information, as requested. <br /> <br />A reporting form will be provided to each county requesting this operating assistance with the <br />county,s disbur_sement.
