Section 16 - Post Rehabilitation Return to Work Agreement: <br /> <br /> A. I understand that my previous job performance warrants close supervision for <br />an extended period of time upon my return to work and I will accept such supervision as a <br />constructive part of my recovery. <br /> <br /> B. I understand that upon. my return to work, I must meet all established standards <br />of conduct and job performance and that I will be subject to the Company's disciplinary <br />procedures for any failure to meet those standards. <br /> <br /> C. For a period of one (I) calendar year after the date of reinstatement to my <br />position, I will be subject to random drug and alcohol tests at the sole discretion of the <br />Company. The Company shall pay the cost of such tests as well as all time spent by me in <br />submitting to such examinations. <br /> <br /> D. I agree that I shall cooperate fully with any and all Company requests to <br />submit to any random drug and alcohol tests. I further acknowledge and understand that <br />if I fail to cooperate fully or to submit to such drug tests when requested, such failure will <br />be cause for discipline under this policy. <br /> <br /> E. In the event that any of my drug or alcohol tests indicate a positive reading <br />which discloses that I have used any of the prohibited substances or alcohol identified in <br />Section 1 of this Policy, I will be subject to discipline up to and including discharge. <br /> <br /> F. I understand that I will be subject to the above stated rules and conditions for <br />twelve (12) calendar months. <br /> <br />I UNDERSTAND AND AGREE THAT MY LEAVE OF ABSENCE, <br />REINSTATEMENT, AND CONTINUED EMPLOYMENT ARE CONTINGENT <br />UPON MY SATISFYING ALL OF THE ABOVE RULES AND CONDITIONS OF <br />EMPLOYMENT AND THAT MY FAILURE TO DO SO SUBJECTS ME TO <br />DISCIPLINE, UP TO AND INCLUDING DISCHARGE. <br /> <br />SIGNATURE DATE <br /> <br />SUPERVISOR/DIVISION MANAGER <br /> <br />-9- <br /> <br /> <br />
