Page 8

PLACEMENT AND PAYMENT PROCEDURES

1. Referrals of delinquent, undisciplined, or at risk males ages 10 - 17 from
Cabarrus County may be made by Court Counselors, Social Workers, school officials,
or Mental Health professionals according to Uwharrie Homes, Inc. written referral
procedures.

2. Decisions concerning admissions are made by an admissions committee.
This committee consists of representatives from juvenile services, DSS, mental health,
city and county schools and Uwharrie Home staff. Admissions of children to the group
home will be made according to the written admissions procedures.

3. Children approved for placement will either enter the treatment program
immediately, or be placed on a waiting list pending an opening. Treatment at Uwharrie
Homes is up to one year, but averages around 6-8 months.

4. Cost of placement (contracted services) will be:
a, $360 per month per child if the child receives services at the group
home for at least 24 days during a given month.

b. $12 per day if a child receives services at the group home for less
than 24 days during the month.

5. Uwharrie Homes, Inc. will bill Cabarrus County on a monthly basis. Payments
will be in lieu of parents payments. Parents/guardians will be billed only if the total
amount of CBA funds has been used and a child is still in need of placement.

6. The amount of CBA funds requested could fund approximately 24 months of
placement at Uwharrie Homes, Inc. ($6646 CBA and $1994 Cabarrus County match).

J~-Earnlar~, Execrative Director
