]C,~I]ARI~US COUNt] <br /> <br />CAROLINA <br /> <br />METHODOLOGY TO ADDRESS SERVICE NEEDS OF <br /> LOW-INCOME MINORITY ~!.D~RLY <br /> <br />OBJEC~ I: To ensure the delivery of services to provide to the identified low income <br /> minority elderly in Cabarrus County. <br /> <br />Cabarrus County Department of Aging provides congregate meals, transportation and in-home <br />aide services to all older adults requesting services regardless of race, sex or economic status. <br />The department makes particular efforts to assure that the low income minority elderly are <br />served through the following strategies: <br /> <br />A. Maintain statistics on low income minority elderly clients enrolled in all service <br /> programs. <br /> <br /> Continue outreach in low income minority ~ommunities utilizing all available resources <br /> (churches, non-profit service agencies, etc.). <br /> <br />C. Provide evaluation tools to evaluate program services and collect input to make <br /> improvements in providing services. <br /> <br />OBJECTIVE H: Maintain awareness for special needs of the low income minority elderly. <br /> <br />Maintain a cooperative relationship with local minority organizations, businesses, <br />churches, etc. in identifying those we are not serving. <br /> <br />Maintain congregate mealsites in minority communities of Concord and Kannapolis and <br />continue to provide nutritional education programs, health screening services and <br />info,,ational programs of other services available. Continue providing transportation <br />to the congregate sites as well as transportation to medical/doctor appointments, human <br />service agencies, senior center activities and shopping. <br /> <br /> Department of Aging <br /> <br />-' - l:, i'?C5 · :_'.2~.:"::...
