Page 2 <br /> <br />SiX MONTHS MEASURABLE OBJECTIVES STATUS <br /> <br />(Continuation Programs Only) <br /> <br />In the space provided, please list each measurable objective in your current <br />program agreement and indicate the degree to which your program has been <br />successful in achieving them. (Use data from the first six months of the <br />fiscal year.) <br /> <br />Measurable objectives met July l, 1993 thru December 31, 1993. <br /> <br />85% referred children served <br /> 90% - 10 of 11 children referred were served. One was referred when <br /> home was full. Three were already in placement on 7/1/93 <br /> giving a total of 13 served during first six months. <br /> <br />60% prepared for long term placement <br />77% - 10 of 13 returned home, placed in foster home or group home or <br /> placed with relatives as determined to be the best long term <br /> placement. <br /> 1 placed in psychiatric hospitalization. <br /> 1 was short-term and removed by court. <br /> <br />50% achieved successful adjustment for 6 months <br />69% - ll of 16 children placed between September 1, 1992, and March 31, <br /> 1993 have had no new court referrals and have made successful <br /> long term care placements. One of the 16 children was terminated <br /> to "Willie M" services for placement. Two had had multiple <br /> placements. One-went to training school. Of the 11 successfully <br /> adjusting to their placements, 4 are at home. <br /> <br /> Between 7/1/92 and 6/30/93, we had 26 children placed in the group <br />home; 4 were for less than 30 days, 8 for 30 to 60 days, 2 for 60 to 90 <br />days, and 12 for the maximum 90 days. If we have 5 children stay for 90 <br />days our maximum is 20 children served. <br /> <br />From 7/1/93 thru 2/11/94, we have had 17 children placed. Currently <br />there are 4 children placed in the group home as of 2/11/94. <br /> <br /> In the CBA 1st quarter report of FY-94, our statistics for the past 12 <br />months showed 65% referred by court, 85% referred as delinquent or status <br />offenders. Our performance indicators showed 82% decrease in additional <br />court referrals, 89% decrease in runaways, and 36% decrease in school <br />suspension/expulsion. 75% were between the ages of 14 and 17; 30% were <br />White, 60% Black; 65% female, 35% male. 65% were active with Juvenile <br />Court. 35% were on probation or court supervision, 20% had a petition <br />filed, 5% were diverted from court and 30% were at risk of Juvenile Court <br />involvement. 80% had successful completion of the program. 4% were removed <br />by court, 4% because of runaway and 8% did not adjust. At time of discharge <br />88% were enrolled in school, 8% had had court referrals while placed at the <br />group home, 16% had had at least one runaway and 4% had been in secure <br />custody. At discharge 36% went to relatives or back home, 60% went to group <br />home, 4% went to training school. <br /> <br /> <br />
