Page 3.1 <br /> <br />MKASURABLE OBJECTIVE(S): <br /> <br />1. Provide an emergency residence for 85% of all children determined <br /> appropriate for placement. <br />2. To provide a licensed group home for 5 children supervised by Lutheran <br /> Family Services staff. <br />3. All youths will be in DSS custody. <br />4. All youths will have medical examinations within 10 days of placement. <br />5. All youths will have psychological evaluation unless recent (within the <br /> last six months) prior to placement. <br />6. Ail youths experiencing substance abuse will be referred to substance <br /> abuse screening and follow up counseling when indicated. <br />7. All youths will be enrolled in a school system and educational assess- <br /> ments will be done with the school to determine the most appropriate <br /> educational setting for the child. <br />8. 60% of all youths served by the program will be prepared for long-term <br /> placement within 90 days of the entry date (preparation is defined as <br /> stabilization of the child's behavior to a point where he is ready for <br /> placement and development of a long-term care plan). <br />9. Of the children completing the 90 day program, 50% will achieve <br /> successful adjustment for six months following long-term placement. <br /> (Success shall be defined as: [1] The child does not come to the <br /> attention of the Juvenile Court; [2] The child, the social worker, and <br /> his legal guardian agree that placement was successful six months after <br /> it was made.) <br /> <br />INFORMATION MAINTAINED FOR EFFECTI%*ENESS MEASUREMENT: <br /> <br />1. Child's file will be maintained at the group home containing admission <br /> form, student intak~ record, parental consent for non prescription <br /> medication, social summary, student visitation forms, plan of care <br /> forms, staff log sheets regarding each child. <br />2. Department of Social Services will keep a folder on each child <br /> containing court order showing custody with Department of Social <br /> Services, termination summary on the child, case dictation involving <br /> family counseling and coordination of services to the child and far~ily, <br /> monthly progress reports submitted by the home to DSS, client tracking <br /> forms submitted to CBA office. <br />3. Budget information documenting in-kind and cash flow kept by both the <br /> home and DSS. <br />4. Staff work schedule kept by the home. <br /> <br /> <br />
