AGREEMENT FOR PROVISION OF PSYCHOLOGICAL SERVICES <br /> TO JUVENILE SERVICES <br /> <br /> This agreement is made and entered into this 1st day of July <br />1994, by and between Juvenile Services Division 19 A of the <br />District Court District (hereinafter referred to as Juvenile <br />Services), Piedmont Area Programs Cabarrus Center (hereinafter <br />referred to as Piedmont), and Cabarrus County for the provision of <br />professional psychological services to the youth and families <br />involved in the Juvenile Court System in Cabarrus County. These <br />services consist of two Components hereinafter referred to as <br />Component A and Component B. <br /> <br />Piedmont agrees to the following: <br /> <br /> 1). In regard to Component A, Piedmont shall provide of the <br />following described services at the below-described times and <br />places: <br /> -Contract services provided by a number of Cabarrus <br />Family Recovery and Cabarrus Children's Services <br /> clinicians. These services will include: <br /> <br /> # time per hour <br />Psychological Evaluations 6 3hr $60 <br />Psychiatric Evaluations 5 lhr $75 <br />Individual Therapy 75 lhr $60 <br />Family ?herapy t0 lhr $60 <br />Group Therapy 30 1.Shr $26.50 <br />Substance Abuse <br />Assessment & Treatment ti0 1.Shr $26.50 <br />Drug Screening 16 $30 <br />Consultation <br />Referral <br />Case Management <br /> <br />est. cost <br />$1080.00 <br /> 375.00 <br /> 4500.00 <br /> 600.00 <br /> 1190.00 <br /> <br />4370.00 <br />480.00 <br />0.00 <br />0.00 <br />0.00 <br /> <br />$12,595.00 <br /> <br />-Approximately 60-90 youth will receive services through <br />this contract. <br /> <br /> 2). With regard to Component B, Piedmont shall provide a part <br />time position which will provide psychological services including <br />screenings, psychological evaluations, individual, group and family <br />therapy, youthful sex offender evaluations and treatment, as well <br />as consultation with juvenile court counselors and referral and <br />coordination for specialized services. This position will serve <br />approximately 90-120 youth. It is understood that some youth will <br />receive services through both Components A and B. <br /> <br /> 3). in both Components A and B, clinicians providing services <br />shall see new referrals for their services in a timely manner.
