the provision of test results to Juvenile Court shall not exceed 20 <br />days. <br /> <br /> 4). Piedmont agrees to submit to the Cabarrus County Finance <br />Officer a monthly invoice of services rendered to each client <br />through Component A. A separate monthly invoice Wil! be submitted <br />for Component B. The invoices will be forwarded to Juvenile <br />Services for initialing prior to submission to the county. <br /> <br />Juvenile Services agrees to the following: <br /> <br /> 1). To provide a written referral which includes required CBA <br />tracking information. <br /> <br /> 2). To provide relevant records and reports i.e., school <br />records, police reports, social histories. <br /> <br /> 3). To review and forward all invoices for payment from <br />Piedmont to Cabarrus County Finance Officer on a monthly basis. <br /> <br />Both Piedmont and Juvenile Services agree to the following: <br /> <br /> 1). To adhere to all CBA standards for psychological <br />services to the Juvenile Court. <br /> <br />Cabarrus County agrees to the following: <br /> <br /> 1). With regard to Component A Cabarrus County shall pay <br />Piedmont at the rate of (the amount billed will vary depending upon <br />services rendered) per month with the maximum total amount payable <br />under this contract being $12,606.00. <br /> <br /> 2). With regard to component B Cabarrus County shall pay <br />Piedmont at the rate of $952.75 per month with the maximum total <br />amount payable under this contract being $11,433.00. <br /> <br />This agreement shall remain valid until June 30, 1995, or within 30 <br />days of written notice by either party. <br /> <br />Esther S.
