THE CONCORD TELEPHONE COMPANY <br /> <br />LEASE AGREEMENT <br /> <br />This Lease Agreement is made and entered into between The Concord <br />Telephone Company, a North Carolina Company, with its principal <br />office in. Concord, North Carolina, hereinafter called LESSOR, and <br /> <br />Name of Firm <br /> <br />Cabarrus County Home Health Aqency <br /> <br />Street and Address 28 Branchview Drive NE <br /> <br />Concord Cabarrus <br /> <br />City County <br /> <br />N.C. 28025 <br /> <br />State Zip Code <br /> <br />hereinafter called LESSEE. <br /> <br />In consideration of the mutual agreement herein contained LESSOR <br />agrees to design, engineer, install and maintain the communications <br />system described in Schedule A and LESSEE hereby authorizes LESSOR <br />to design, engineer and install said communications equipment.. <br /> <br />TERMS AND CONDITIONS <br /> <br />1. TERMS <br />The LESSOR agrees to lease to the LESSEE the communications <br />equipment as specified on Schedule A hereto annexed for a period <br />of sixty (60) months with the option to renew at the end of the <br />initial sixty (60) month term as hereinafter set forth. Said term <br />shall commence on the date of completion of installation of said <br />equipment as per Paragraph 3 and continue in effect for a period <br />of sixty (60) months thereafter. <br /> <br />2. EQUIPMENT LEASE AND INSTALLATION <br />The LESSOR agrees that all equipment listed in the annexed Schedule <br />A which is made a part of this lease is to be specially constructed <br />for the benefit of LESSEE and installed at the following location: <br /> <br />28 Branchview Drive NE, Concord, N.C. <br /> <br />Installation shall include all the necessary cable, wire, hardware, <br />etc., including labor for the installation of the system (except <br />the cost of concealing wires, furnishing or installing rigid or <br />flexible conduit or wiremold or the extra cost attributable to <br />having to have the installation made by a contractor other than the <br />LESSOR's contractor). LESSEE agrees to supply as many grounded <br />A.C. outlets of type specified by LESSOR as shall be required to <br />power the control equipment and amplifiers (including <br />switchboards), in addition to appropriate environmental and <br />physical space requirements. <br /> <br /> <br />
