DOA-730 <br /> (Rev. 1/94) <br /> <br />Home and Community Care Block Grant for Older Adults <br /> <br /> County Funding Plan <br /> <br />Identification of Agency or Office with Lead Responsibility for County Funding Plan <br /> <br />County Cabarrus <br /> <br />July 1, 1994 through June 30, 1994 <br /> <br />The agency or office with lead responsibility for planning and coordinating the County <br />Funding Plan recommends this funding plan to the Board of Commissioners as a <br />coordinated means to utilize community-based resources in the delivery of comprehensive <br />aging services to older adults and their families in Cabarrus County. This <br />funding plan was presented at a public planned use hearing on May 16 ,1994 <br /> <br />Cabarrus County Dept. of Aging <br /> <br />(Name of agency/office with lead responsibility <br /> <br />~thorized signature ' /(date) <br /> <br />Eleanor B. Koski, Director <br /> <br />(Typed name and title of signatory agent) <br /> <br />(Attach minutes of planned use hearing) <br /> <br />For each service specified on the County Services Summary (DOA-731), or the <br />Provider Services Summary (DOA-732), provide an attachment with reasons for <br />exceeding the average net unit cost of the service in the county for the prior state <br />fiscal year, plus the rate of annual cost of living increase as determined by the State <br />Office of Budget and Management. <br /> <br /> <br />
