Local Health Department Directors <br />November 30, 1992 <br />page 2 <br /> <br /> Your consolidated contract amendment, activities budget, and <br />contract addendum are attached for your review and signatures. <br />This year's award is consistent with the immunization component of <br />the Communicable Disease needs formula that was approved by the <br />Health Director's Association. You can expect a similar level of <br />funding in October, 1993. However,~h~'reve-~of-fun~i~g~-~~ <br />w~ eb~bas--~ on our success in achieving the goal of age- <br />appropriately immunizing 90 percent or more of our two-year-olds. <br /> <br /> Your progress toward increasing the immunization levels of <br />preschool children will be measured by a before-and-after estimate <br />of the number of DTP #4 doses (i.e., the fourth dose of DTP vaccine <br />as a surrogate for a complete immunization series) administered to <br />children prior to their second birthday. Your county's DTP #4 <br />baseline estimate for a 12 month period is listed in the contract <br />addendum. For grant purposes, you will need to specify the <br />projected increase in 4th dose DTP that you will administer during <br />a 12 month evaluation period, that begins on April 1, 1993 and ends <br />on March 31, 1994. Please see the attached contract addendum for <br />details about the evaluation methods. <br /> <br /> The primary intent of this fundinq is to increase aqe- <br />.appropriate immunization of two-year-olds throuqh establishinq more <br />immunization clinics in more locations, that are open on more days <br />and with lonqer hours. It is intended to assist you in: providing <br />more accessible immunization services; removing barriers to <br />immunization; reducing missed opportunities to vaccinate; <br />effectively tracking and following-up on children who are behind on <br />their immunizations; measuring immunization coverage; raising <br />awareness among the'public, parents, and providers and developing <br />innovative approaches to improving immunization levels in <br />vulnerable children. This funding may not be used for operations <br />research, facility construction, vaccine purchase, or disease <br />surveillance activities. <br /> <br /> We want you to be creative and develop a county-wide action <br />plan that meets the unique needs of your clientele. We encourage <br />you to augment this funding by considering: <br /> <br />>>combining your award with that of neighboring counties and <br />implementing joint immunization activities and/or sharing <br />staff; <br /> <br />>>expanding the hours and roles of outreach workers, such as <br />those associated with EPSDT, maternity and child services <br />coordination to include follow-up for all preschoolers whose <br />immunizations are not up-to-date; <br /> <br /> <br />
