ATTACHMENT "A" <br /> <br />IMMUNIZATION ACTION PLAN (IAP) <br /> AID TO COUNTIES <br /> <br />CABAI~RUS COUNTY <br /> <br />AMOUNT OF AWARD: <br /> <br />Your county's award for the period from December 1, 1992, through <br />September 30, 1993, is $ 7,722. <br /> <br />HOW CAN THESE FUNDS BE SPENT: <br /> <br />Funds may be spent in the Salary and Fringe (SA/FR), Operational <br />Expenses (OP EXP), Equipment (EQUIP), and General categories. <br /> <br />FUNDING PERIOD: <br /> <br />This year's funding period will run from December 1, 1992, <br />through September 30, 1993. Health Departments may elect to <br />receive these funds in one of two ways: <br /> <br /> /1) Health Departments that are planning to use IAP funds <br /> ~.~.~% to pay for salary and fringe will receive 70 percent of <br />7~ ~.~ their award to cover the period from December 1, 1992, <br /> ~0~.~ through June 30, 1993. The remaining 30 percent will <br /> ~.~%.~e% be held in reserve and reawarded to the counties after <br /> July 1st to cover the period from July 1st through <br /> <br />September 30, 1993. <br /> <br />Health Departments that are not planning to use IAP <br />funds to pay for salary and fringe may elect to spend <br />the entire award by June 30, 1993. However, additional <br />IAP funding Will not be available until October, 1993. <br /> <br />PLEASE NOTE: TO ENSURE THAT ALL IAP MONIES ARE ENCUMBERED DURING <br />CALENDAR YEAR 1992, WE ARE ASKING EACH COUNTY TO USE A DECEMBER <br /> <br />1, 1992, DATE ON THE ENCLOSED BUDGET FORMS. <br /> <br />PLEASE SIGN AND RETURN THE ENCLOSED BUDGET MATERIALS AND CONTRACT <br /> <br />ADDENDUM BY JANUARY 20, 1993. <br /> <br /> <br />
