Negotiable Objectives and Required Items: <br /> <br />l) <br /> <br />Baseline vaccine doses administered information between July 1, <br />1991 through June 30, 1992, shows that your health department <br />administered 59] doses of DTP #4 to children prior to <br />their second birthday. <br /> <br />2) <br /> <br />Between April 1, 1993 and March 31, 1994, your health department <br />will increase to 864 the number 'of DTP #4 doses <br />administered to children prior to their second birthday. <br /> <br />This represents a 25 percent increase from number of DTP f4 <br />vaccine doses administered in 1991-92 (The percent increase <br />should be a minimum of 10 percent). <br /> <br />3) <br /> <br />By April 1, 1993, the local health department will submit a brief <br />plan, to the Immunization Branch, that outlines your health <br />department's proposed activities to increase the number of <br />vaccine doses administered to children prior to their second <br />birthday. This brief plan should include a description of your <br />health department's barriers to immunization and your plans to <br />overcome these barriers. At a minimum, this plan should discuss: <br /> <br />a) <br /> <br />Provisions for walk-in immunization services, or the <br />expansion of current walk-in hours. <br /> <br />b) <br /> <br />Plans to expand the number of sites, both public and <br />private, that offer immunizations in-your county. <br /> <br />c) <br /> <br />If an appointment system is used, include plans to <br />allow clients to schedule appointments within one week <br />of their request. , <br /> <br />d) <br /> <br />Plans to minimize the time clients have to wait during <br />an immunization visit.
