However, <br />children seen for "immunizations only" visits can be <br />referred for appropriate well child care, and if possible or <br />appropriate, seen by the EPSDT, CSC, or other care <br />coordinator while in the health department. <br /> <br />Each health department should have signed standing orders <br />for appropriate immunizations and physician referrals when <br />contraindications exist. The standing orders should not <br />contain inappropriate contraindications such as mild <br />illnesses or treatment with antibiotics. The standing <br />orders should_comply with the-immunization schedule <br />recommended by the Immunization Bran6h-and should require <br />simultaneous administration of needed vaccines. <br /> <br />8) <br /> <br />9) <br /> <br />All v~cci~':~ ~d~f: ~stra+~cr: should be documented in the <br />patient's permanent medical record, including vaccine type, <br />date, dose, manuiactu~er and lot nun. bet. Vaccine doses <br />administered should be entered into the computer database <br />and reported monthly to the Immunization Branch. <br /> <br />Adverse events should be recorded and submitted to the <br />Immunization Branch. <br /> <br />s0) <br /> <br />Each health department should maintain a system to track <br />immunizations and conduct follow-ups to remind patients of <br />scheduled and/or missed vaccinations.
