Interagency Relations <br /> Serve as a liaison to community agencies. <br /> Maintain contact with agencies regularly to coordinate Case <br /> Management/Service delivery. <br /> Identify resource persons in community agencies to serve as contact <br /> persons in service areas. <br />BI. Case Advocacy/Referral <br /> Provide professional problem assessment/solving and advocacy work to <br /> identified clients. <br /> Appropriately respond to incoming requests and follow-up when indicated. <br /> Assist with walk-in clients. · <br /> Assist with opening and closing cases to assure linkage with appropriate <br /> services. <br /> Responsible for establishing a service plan, including continuity for <br /> interagency/multi-agency service involvement, for each client. <br /> IV. Other duties as assigned. <br /> <br />Master's Degree in social work or related fields. Three years experience <br />in direct services and at least one year indirect service delivery <br />management. Strong knowledge and skills in client advocacy. Good oral <br />and written communication skills. Ability to work with the general public, <br />agency professionals and volunteers. <br /> <br /> <br />
