Page 2 <br />Cabarrus County <br />April 16, 1993 <br /> <br />Aging Activities <br /> <br />Home and Community Care Block Grant Services (HCCBG) <br /> <br />Implemented the 1993 Region F Conference on Aging. <br /> <br />Facilitated two meetings and developed materials for the Home and Community <br />Care Block Grant Advisory Committee to continue working on the county's FY'94 <br />Aging Funding Plan. <br /> <br />Conducted a training session for the Division of Aging for Regions F and E on <br />changes to the Service Cost-Sharing policies and instructions for the FY'94 County <br />Aging Funding Plans. <br /> <br />Attended the "Symposium on Aging: Toward a Unified Social and Health Services <br />System for Older Adults and Their Families" hosted by the Institute of Government <br />at the University of North Carolina at Chapel Hill on March 24, 1993. <br /> <br />Completed ALL ABOUT AGING - No. 9 (April, 1993), the Area Agency on Aging's <br />monthly informational directive to all local community service providers. <br /> <br />Received and processed Management Information System (MIS) monthly reports <br />at Centralina and submitted to the Division of Aging. <br /> <br />Senior Community Service Employment Program <br /> <br />Entered and/or updated enrollee information and prepared Monthly Progress <br />Report to the Division of Aging. <br /> <br />Long Term Care Ombudsman Program <br /> <br />Provided orientation to new Nursing Home Community Advisory Committee <br />members. <br /> <br />Organized in-facility orientation for new Nursing Home Community Advisory <br />Committee members. <br /> <br />Worked with residents, families, and staff to resolve grievances. <br /> <br />- Facilitated meeting for the Association of Long Term Care Social Workers. <br /> <br />Presented information on Senate Bill 33 to the Cabarrus Legislative breakfast. <br /> <br /> <br />
