SIX MONTHS MEASURABLE OBJECTIVES STATUS <br /> <br />(Continuation Programs Only) <br /> <br />In the space provided, please list each measurable objective in your current <br />program agreement and indicate the degree to which your program has been <br />successful in achieving them. (Use data from the first six months of the <br />fiscal year.) <br /> <br />Measurable objectives met July 1, 1992 thru December 31, 1992. <br /> <br />85% referred children served <br /> 87% - 13 of 15 children referred were served. Two were on waiting list <br /> and were admitted as soon as vacancies occurred. Four were <br /> already in placement on 7/1/92 giving a total of 17 served during <br /> first six months. <br /> <br />60% prepared for long term placement <br />67% - 8 of 12 returned home, placed in foster home or group home or <br /> placed with relatives as determined to be the best long term <br /> placement. <br /> 2 certified "Willie M" and needed further treatment to stabilize. <br /> 1 placed at home but disrupted and was placed in group care. <br /> 1 was short-term and removed by court. <br /> <br />50% achieved successful adjustment for 6 months <br />67% - 12 of 18 children placed between March 1, 1992 and September 30, <br /> 1992 have had no new court referrals and have made successful <br /> long term care placements. Two of the 18 children were terminated <br /> to "Willie M" services for placement and have continued to have <br /> repeated severe behavior problems including court referrals. Two <br /> reached age 16 and have adult court charges. Of the 12 <br /> successfully adjusting to their placements, 5 are at home. <br /> <br /> Between 7/1/91 and 6/30/92, we <br />home; 7 were for less than 30 days, <br />days, and 9 for the maximum 90 days. <br />our maximum is 20 children served. <br /> <br />had 26 children placed in the group <br />3 for 30 to 60 days, 7 for 60 to 90 <br />If we have 5 children stay for 90 days <br /> <br />From 7/1/92 thru 3/12/93, we have had 21 children placed. Currently <br />there are 5 children placed in the group home as of 3/12/93. <br /> <br /> In the CBA 1st quarter report of FY-93, our statistics for the past 12 <br />months showed 85% referred by court, 95% referred as delinquent or status <br />offenders. Our performance indicators showed 80% decrease in additional <br />court referrals, 83% decrease in runaways, and 59% decrease in school <br />suspension/expulsion. 75% were between the ages of 14 and 17; 45% were <br />White, 55% Black; 65% female, 35% male. 85% were active with Juvenile <br />court. 80% were on probation or court supervision and 10% were at risk of <br />Juvenile Court involvement. 84% had successful completion of the program. <br />10% were removed by parents and 5% did not adjust. At time of discharge 94% <br />were enrolled in school, 31% had had court referrals while placed at the <br />group home, 21% had had at least one runaway and 5% had been in secure <br />custody. At discharge 57% went to relatives or back home, 36% went to group <br />home, 0% went to training school. <br /> <br /> <br />
