D. Prepare ~nd sub~t a statistical perfo~nance progress report monthly for <br />presentation to the County, showing the number of agengiea, participating, <br />].niles driven, passengers serviced, systan cost for the month, total number of <br />client trips per day ~nd per month, fuel consumed (gallons), total fuel cost, <br />and average fuel,cost per gallcn, reimburs~nlent rate per.mile, total system <br />cost per day ~nd per month, and the system cost per client trip; Provide the <br />County with sunn~ries on a periodic basis; assist in the completion of <br />funding re.cuests (~TA Grants) ~d annual minor TDP updates; <br /> <br />E. Prepare and sub~dt quarterly reports required by the North Carolina <br />Department of Transportation; <br /> <br />F. On ~n on-going basis, evaluate the perfozmrance of the system for the <br />purpose of identifying potential operational and passenger hatching <br />improve-nents, end assist the County in imp]amenting the recon/nendations of the <br />LOCAL AEEATDP; <br /> <br />G. Provide transportation only to authorized passengers in the system; Bill <br />the County on a monthly basis, utilizing an it~nized invoice; <br /> <br />H. Make avail-~ble any arid all records and written materials pertaining to <br />client use statistics, dispatch activities ~nd vehicle data to the County upon <br />request; <br /> <br />I. Ensure that specified Ii-~bility, collision, ~nd comprehensive ir~ur~nce <br />as specified in the County's RFP and CTS's proposal is in force on all <br />vehicles; and hold harmless the County and its officers, ~nployees, and agents <br />fremevery claim, d~-nand, liability, loss, damage, or expense o~ any nature <br />whatsoever, and all cost or expenses incurred in connection therewith wP~ch <br />arises at any time by reason of damage to the property of or personal injury <br />to any person occurring or arising out of the performance by CTS officers, <br />~mployees, or agents, occasioned by any actual negligent act or omission by <br />CTS, except as specified in ~-tic]e I~, Sections H ~nd I; <br /> <br />J. Ira~-~diately notify the County of any accidents or i3ncidents involving <br />any vehicles and clients, whether or not damage or injury results, and ~ake a <br />detailed report of the accident or incident to the director of the agency <br />whose clients were involved, ~nd as required to the North Carolina Department <br />of Transportation; <br /> <br />K. Adhere to the holiday schedule adopted by the County of C~barrus for its <br />County offices; <br /> <br />L. For the purpose of weather cancellations, follow the local School Board <br />policy for closings unless otherwise directed by the County. <br /> <br />M. Renegotiate the rate if the average service miles consistently falls <br />be]ow or rises above 1/12 of 350,000 per annum. <br /> <br />N. Furnish additional vehicles to the County upon request at an additional <br />rate to be added to the rates herein contained.
