Box 707 <br />77 Union Street <br />Concord, NC 28025 <br /> <br />I-DR <br /> <br />Re: Coddle Creek Reservoir <br /> Blythe Industries Inc. <br /> Proposed Change Orders Nos. 6, 7 & 8 (A00700) <br /> HDR Project No. 00187-034-018 (CA) D1 <br /> <br />Dear Mr. Witherspoon: <br /> <br />Three change orders have been negotiated with Blythe Industries on the Dam and Reservoir contract <br />for adjusting bid item quantities, unit prices, and addition of items of work required for the project. <br />The change orders have been accepted by the Contractor and are recommended for approval by the <br />Engineer, and if approved, should be executed by the Owner to become a part of the Contract. The <br />change orders are summarized below: <br /> <br />Change Order No. 6 adjusts the unit price for Zone C material (sand drainage blanket) <br />from $15.00 to $19.95 (an increase of $4.95). Locally available material meeting the <br />specifications would not perform as intended, requiring the material specifications to be <br />modified to ensure a working drainage blanket. The unit price was increased for <br />additional haul distance and stockpiling costs. Net increase of $152,955.00 <br /> <br />2. Change Order No. 7 adds four pay items to the Part A, Dam portion of the Contract. <br /> <br />Pay Item 45 provides additional mobilization and related items for additional <br />work required for successful completion of the Grout Curtain. Net increase <br />of $8,537.03. <br /> <br />b) <br /> <br />Pay Item 46 adds a pay item for the Grout Curtain for drilling through Zone <br />A Material and overburden. This resulted from the need to modify the Phase <br />II Grout Curtain depth and sequence of construction to obtain optimum results. <br />Net increase of $20,617.50. <br /> <br />c) <br /> <br />Pay Item 47 reimburses the Contractor for additional engineering costs <br />required to meet OSHA standards necessitated by the modifications to the <br />Phase II Grout Curtain and Excavation portions of the project.
