PHYSICIAN' S STATEMENT <br /> <br />The facility will assist EMS in getting a completed Physician's Statement <br />specifying the need for ambulance transportation. 'This form will enhance <br />Medicare reimbursements. It will also help EMS determine when a facility <br />is abusing the system and should be billed directly for the services. <br /> <br />TRANSPORT OF NON-EHERGENCY CONV~T.~--~-NTS <br /> <br />EMS will transport all non-emergency convalescent calls to Union Memorial <br />Hospital after 6:00 p.m. Transports of non-emergency nature outside Union <br />County will be authorized by the Emergency Room Physician or Union Memorial <br />Staff Physician after 6:00 p.m. <br /> <br />GUIDELINES ADOPTED BY UNION COUNTY BOARD OF COMMISSIONERS MAY 21, 1990 <br /> <br /> <br />
