CONTRACT <br /> <br />This agreement is made and entered into this the 1st day of January., 1993 between the <br />Mecklenburg County Health Department and the Cabarrus County Health Department for <br />services as described below: <br /> <br />A. Scope of Services <br /> <br />The Mecklenburg CoUnty Health Department agrees to provide mammography screening <br />for the.clients of the. Cabarrus County Health Department.~ Marnmography results will be <br />mailed to the patient's primary physician for appropriate follow-up. <br /> <br />B. Period of Agreement <br /> <br />This agreement shall be effective .on the date of execution and may be amended or <br />terminated at the mutual consent of both parties or terminated by either party giving 30 <br />days written notice. <br /> <br />C. .Pa.vrnent of SerVices <br /> <br />(1.) <br /> <br />Rennmeration for services shall be at the rate of $58.00 per mammogram. <br />Fee subject to adjustment annually to match fee schedule approved by the <br />Mecklenburg County Board of Commissioners. Fee also subject to <br />adjustment to comply with Medicaid regulations should Mecklenburg County <br />Health Department receive State approval to bill Medicaid for mammography <br />services. <br /> <br />(2.)
