Page ~ of A <br /> STATE OF NORTH CAROLINA AMENDMENT TO CONTRACT <br /> <br /> Amendment Number i to Contract, CoDtract for services Provided for <br /> Under Title III of the Older Americans Act and/or Social Services Block <br /> Grant Funds and or ~orth Carolina State Grant Funds <br /> This Amendment, entered into this 2?th day of February , <br /> <br /> 1991 , by and between the Centralina Council of Governments (hereinafter <br /> <br /> called the "Council") and Cabarrus County (foF services provided by its <br /> <br /> Department of ~q~nq) (hereinafter called the Agency"). <br /> <br /> WITNESSETH T~AT; <br /> <br /> WHEREAS, the Council and the Agency entered into that certain agreement <br /> <br /> dated July ~, 1990 pursuant to which the Agency was'to <br /> <br /> provide certain services for persons 60 years of age and older; and <br /> <br /> WHEREAS, certain changes may need to be made in projections of units of <br />service to be provided in order to recognize past performance as well as <br />needs for the remainder of the contract period; and <br /> <br /> WHEREAS, certain changes may need to be made in the AAA unit rates; and <br /> WHEREAS, certain changes may need to be made in the total grant funds; <br /> <br /> NOW, THEREFORE, it is agreed that the aforesaid Contract be and it is <br />hereby amended in the following respects: <br /> <br /> 1. Paragraph 2 of the Contract is amended by deleting therefrom the <br /> following services, m~her of units, AAA unit rates and total grant <br /> funds, <br /> <br /> <br />
