CONTRACT FOR INSTITUTIONAL RESPITE CARE SERVICES <br /> <br /> THIS AGREEMENT, entered into as of this 1st day of July, 1991 by and between <br /> First Assembly Adult Day .Care (hereinafter referred as "Center" and Cabarrus <br /> County Department of A~in~, (hereinafter referred to as the "Purchaser"). <br /> <br /> WITNESSETH THAT: <br /> <br /> W~S, the Purchaser desire to engage the First Assembly Adult Day Health to <br /> render services in connection with the Institutional Respite Care Services for <br /> the older adults of Cabarrus County (hereinafter referred to as the "Pro, ram"), <br /> described in the l~irchaser's contract with Centralina Council of Governments <br /> designated for the provision of respite care services through Title III funds to <br /> older adults provided to the Purchaser through Centralina Council of Governments <br /> from North Carolina Division of Aging and United States Department of Health and <br /> Human Services, and. <br /> <br /> WH~S~ First Assembly Adult Day Care Center is a licensed adult day care <br /> service provider with the demonstrated capacity and desire to render <br /> institutional respite care services to older adults; <br /> <br /> NOW THEREFORE, in consideration of the premises and the mutual covenants and <br /> agreements hereinafter contained, the parties hereto a§ree as follows: <br /> <br /> 1. The Center agrees to sell and Purchaser agrees to buy Center's respite <br /> services. The Center agrees to provide services and activities in <br /> accordance with standards as provided by N.C. Division of Aging. <br /> (Attached) The Center agrees not to subcontract for the delivery of the <br /> services called for in this Agreement unless mutually agreed by Purchaser <br /> and the Center. <br /> <br />2. This Agreement will be in effect and services will be delivered beginning <br /> July 1, 1991 and continuing through June 30, 1992. <br /> <br /> 3. Purchaser will purchase up to 576 unit hours with no more than A8 hours per <br /> client mouth within the stated contract period. The Center shall be <br /> per <br /> co~uitted to deliver the total units of services requested by the <br /> Purchaser. <br /> <br />~. The Center will deliver the Program based on one (1) hour unit of service. <br /> Progremwill be provided in the adult day health facility. <br /> <br />5. The purchase and delivery price for all units of service ordered by <br /> Purchaser shall be as follows: a unit price of $18,00 per day (with a six <br /> hour minimum stay) along with the responsibility outlined in this contract. <br /> The purchase price shall include all expenses of the service outlined as <br /> follows: <br /> <br /> <br />
