CONTRACT FOR INSTITUTIONAL RESPITE CARE SERVICES <br /> <br /> THIS AGREEHENT, entered into as of this let day of July, 1991 by and between <br /> L.I.F.E. Center (hereinafter referred as "Center" and Cabarrus County Department <br /> of A~in~, (hereinafter referred to as the "Purchaser"). <br /> <br /> ~ITNESSETH THAT: <br /> <br /> WHEREAS, the Purchaser desire to the L.I.F.E. Center to render services <br /> in connection with the Institutional Respite Care Services for the older adults <br /> of Cabarrus County (hereinafter referred to as the "Pro~ram"), described in the <br /> PurchaSer% contract with Centraline Council of Governments designated for the <br /> provision of respite care services through Title III funds to older adults <br /> provided to the Purchaser through Centrallna Co~ncil of Governments from North <br /> Carolina Division of Aging and United States Department o£ Health and Human <br /> Se=vices; and, <br /> <br /> WHEREAS, L.I.F.E. Center is a licensed adult day care service provider with the <br /> demonstrated capacity and desire to render institutional respite care services <br /> to older adults; <br /> <br /> NOW ~ID~REFORE, in consideration of the premises and the mutual covenants and <br /> agreements hereinafter contained, the parties hereto agree as follows: <br /> <br />1. The Center agrees to sell and Purchaser agrees to buy Center's respite <br /> services. The Center agrees to provide services and activities in <br /> accordance with standards as provided by N.C. Division of Aging. <br /> (Attached) The Center agrees not to subcontract for the delivery of the <br /> services called for in this Agreement unless mutually agreed by Purchaser <br /> and the Center. <br /> <br />2. This Agreement will be in effect and services will be delivered beginning <br /> July 1, 1991 and continuing through June 30, 1992. <br /> <br />3. Purchaser will purchase up to 576 unit hours with no more than ~8 hours per <br /> client per month within the stated contract period. The Canter shall be <br /> committed to deliver the total units of services requested by the <br /> Purchaser. <br /> <br />4. The Center will deliver the Program based on one (1) hour unit of service. <br /> Program will be provided in the adult day care facility. <br /> <br />5. The purchase and delivery price for all units of service ordered by <br /> Purchaser shall be as follows: a unit price of $17.00 per day (with a six <br /> hour minimum stay) alonE with the responsibility outlined in this contract. <br /> The shall include all of the service outlined <br /> purchase <br /> price <br /> follows: <br /> <br /> <br />
