CONTRACT FOR RESPITE CARE <br /> ~__ TEMPORARY PLACEMENT <br /> <br /> THIS AGREEMENT, entered into as of the 1st day of July, by and between Giles <br /> Leisure Home (hereinafter referred to as the "Center") and Cabarrus County <br /> Department of Aging, (hereinafter referred to as the "Purchaser"). <br /> <br /> WITNESSETH THAT: <br /> <br /> connection with the Respite Care program for the older adults of Cabarrus County <br /> (hereinafter referred to as the "Program"), described in the Purchaser's <br /> contract with Centralina Council of Governments designated for the provision of <br /> respite care services through Title III funds to older adults provided to the <br />ilPurchaser through Centralina Council of Governments from North Carolina Division <br />of Aging and United States Department of Health and Human Services; and <br /> <br /> WHEREAS, The Center is a licensed domiciliary care service provider with the <br /> demonstrated capacity and desired to render domiciliary services to older <br /> adults; <br /> <br /> NOW THEREFORE, in consideration of the premises and the mutual covenants and <br />il agreements hereinafter contained, the parties hereto agree as follows: <br /> 1. The Center agrees to sell and Purchaser agrees to buy Center's respite <br /> services. The Center agrees to provide services and activities in <br /> il accordance with standards as provided by <br /> N.C. <br /> Division <br /> of <br /> Aging. <br /> (Attached). The Center agrees not to subcontract for the delivery of <br /> services called for in this agreement unless mutually agreed by Purchaser <br />il and the Center. <br /> 2. This agreement will be in effect and services will be delivered beginning <br /> il July 1, 1991 and continuing through June 30, 1992. <br /> 3. Purchaser will purchase up to 24 days per client per contract year or up to <br /> 576 hours per client within the stated contract period. The Center shall <br /> il deliver the total units of services requested by the <br /> be <br /> to <br /> Purchaser at an average minimum rate of 24 hour days or 168 hours per <br /> client. <br />~l 4. The Center will deliver the Program based on an on a one hour unit of <br /> service. Program will be provided in the domiciliary home. <br />I 5. The purchase and delivery price for all units of service ordered by <br /> Purchaser shall be as follows: A unit price of $1.04 per hour for <br /> ambulatory participants and $1.09 per hour for semi-ambulatory <br /> I (This rate is based on the 1991 Special Assistance rates for <br /> pa~ticipants. <br /> Domiciliary Romes which states $754 for ambulatory residents and $790 for <br /> semi-ambulatory based on a 30-day month) with the responsibility outlined <br /> I in this contract.
