CONTRACT FOR RESPITE CARE ~__ TEMPORARY PLACEMENT THIS AGREEMENT, entered into as of the 1st day of July, by and between Giles Leisure Home (hereinafter referred to as the "Center") and Cabarrus County Department of Aging, (hereinafter referred to as the "Purchaser"). WITNESSETH THAT: connection with the Respite Care program for the older adults of Cabarrus County (hereinafter referred to as the "Program"), described in the Purchaser's contract with Centralina Council of Governments designated for the provision of respite care services through Title III funds to older adults provided to the ilPurchaser through Centralina Council of Governments from North Carolina Division of Aging and United States Department of Health and Human Services; and WHEREAS, The Center is a licensed domiciliary care service provider with the demonstrated capacity and desired to render domiciliary services to older adults; NOW THEREFORE, in consideration of the premises and the mutual covenants and il agreements hereinafter contained, the parties hereto agree as follows: 1. The Center agrees to sell and Purchaser agrees to buy Center's respite services. The Center agrees to provide services and activities in il accordance with standards as provided by N.C. Division of Aging. (Attached). The Center agrees not to subcontract for the delivery of services called for in this agreement unless mutually agreed by Purchaser il and the Center. 2. This agreement will be in effect and services will be delivered beginning il July 1, 1991 and continuing through June 30, 1992. 3. Purchaser will purchase up to 24 days per client per contract year or up to 576 hours per client within the stated contract period. The Center shall il deliver the total units of services requested by the be to Purchaser at an average minimum rate of 24 hour days or 168 hours per client. ~l 4. The Center will deliver the Program based on an on a one hour unit of service. Program will be provided in the domiciliary home. I 5. The purchase and delivery price for all units of service ordered by Purchaser shall be as follows: A unit price of $1.04 per hour for ambulatory participants and $1.09 per hour for semi-ambulatory I (This rate is based on the 1991 Special Assistance rates for pa~ticipants. Domiciliary Romes which states $754 for ambulatory residents and $790 for semi-ambulatory based on a 30-day month) with the responsibility outlined I in this contract.
