CONTRACT FOR HOHE MANAGE}IENTAND PERSONAL CARE <br /> <br /> THIS AGP~EHENT, entered into as of this 1st day of July, 1991, by and between the <br /> Cabarrus Department of Social Services (hereinafter referred to as "DSS" and the <br /> Cabarrus County Department of A~ing (hereinafter referred to as the "Purchaser"). <br /> <br /> W~TNESSETH THAT: <br /> <br /> WHEREAS, the Purchaser desizes to engage DSS to render In-Home Aide Services at <br /> Levels I and II for the older adults of Cabarrus County (hereinafter referred to <br /> as "program"), described in ~he _Purchaser's contract with Centrallna Co~tncil of <br /> Governments for funds designated for the provision of services to older adults <br /> provided to the Purchaserthrough Centralina Home Managnment and Personal Care <br /> services to older adults'provided to the Purchaser through Centralina Council of <br /> Goverm~ents from North Carolina Division of Aging and United States Department of <br /> Health and Human Services; and, <br /> <br /> Wit--S, DSS is a public agency with the demonstrated capacity and desire to <br /> render In-Home Aide Services to five (5) older adults; at Level I and to eleven <br /> (11) older adults at Level II; <br /> <br /> NOW TH~FOR~, in ~onsideration of the premises and the mutual covenants and <br /> agreements hereinafter contained, the parties hereto agree as follows: <br /> <br /> 1. DSS agrees to sell and Purchaser agrees to buy In-Home Services at Levels I <br /> and II. DSS agrees to provide service activities in accordance with <br /> standards as provided by North Carolina Division of Aging. (Attached). DSS <br /> agrees to refer clients in need of respite services to the purchaser so as <br /> not to duplicate services. DSS agrees not to subcontract for the delivery <br /> of the services called for in this Agreement unless mutually agreed by <br /> Purchaser, DSS, and Centralina Council of Governments. <br /> <br />2. This Agreement will be in effect and services will be delivered beginning <br /> July 1, 1991 and continuing through June 30, 1992. <br /> <br />3. Purchaser will purchase 1465 units of service for Level I and 3285 units of <br /> service for Level II within the stated contract period pending funding from <br /> Area Agency on Aging. DSS shall be committed to deliver ~he total units of <br /> service requested by the Purchaser within stated contract period. <br /> <br />4. DSS will deliver the Program based on one hour unit of service. Program <br /> will be provided in the home of the client. Selvice activities contingent <br /> upon participant need as appropriate professional determines plan of care <br /> appropriate to Level I and/or Level II Service Standards (See attached). <br /> <br />$. The purchase and delivery price for all units of service ordered by <br /> Purchaser shall be as fellows: a unit price of $5.99 with the <br /> responsibility outlined in this contract. The purchase price shall include <br /> all expenses of the service delivery as defined in the service standards and <br /> outlined below. <br /> <br /> (a) Administrative Requirements: Record maintenance of In-Home Services' <br /> cases to include copies of units of services provided. The units must be <br /> documented by time of day with documentation of allowable level of care <br /> activities. Cabarrus County Daily Time Records are not appropriate <br /> documentation of units of service. Records must be signed by the <br /> participant and in-home aide. <br /> <br /> <br />
