I <br /> I Page 2 of 6 <br /> <br /> I (b) DSS will be for in-home aides to satisfy necdssary training <br /> responsible <br /> requirements in regards to level of care providing and also fo: <br /> on-going supervision. <br /> I (c) DSS shall also meet these additional requirements. <br /> <br /> I 1) Procedures for collecting Program Income. <br /> Z) Current job descriptions for each staff member involved in the <br /> service. <br /> 3) Develop procedures for written evaluation of the service by <br /> I participants (at least annually). Survey participants and maintain <br /> evaluation for purchaser's review. <br /> 4) Training plan for all staff involved with service. <br /> I 5) Grievance procedures for all participants. <br /> 6) Written policy for termination of service <br /> 7) Written policy for reporting changes in participants condition. <br /> i 8) Written plan for how the agency will provide general and se:vice, <br /> specific information and procedures for referral. <br /> 9) Written plan and doctLmentation of outreach activities. <br /> <br /> I (d) DSS is required to provide the following reports to the Purchaser by <br /> the designated date of each month: <br /> <br /> I 1) Completed HIS Client Registration Forms. <br /> 2) Completed requests for reimbursement. <br /> a) Requests for reimbursement shall consist of a s%um~ary sheet <br /> <br /> i prepared by DSS. Back-up documentation will be maintained at <br /> DSS which consists of a signed statement by worker and <br /> participant and/or guardian, unpaid caregiver which state hours <br /> and duties of service. Cabarrus County Time Records are not <br /> I sufficient. <br /> 3) Completed MIS Units of Service Report. <br /> <br /> I 6. ~opulation To Be Served: <br /> Any resident of Cabarrus County who: <br /> (a) is 60 years of age and older <br /> I (b) is unable to out tasks essential to the activities of daily <br /> carry <br /> living and/or instrumental activities of daily living, who ]lave no <br /> responsible person available to perform these tasks and who need the <br /> I service in order to remain at hot~. <br /> 7. DSS agrees to meet with Purchaser one time per quarter (or more frequently <br /> i {f requested) to evaluate In-Home Aide Service program operation and quality <br /> ~ of service. In addition an annual audit will be conducted to monitor <br /> compliance of service 'standards. Meetings shall be scheduled at the <br /> convenience of the Purchaser and DSS. <br /> <br />~ 8. All referrals for the direct service provision shall be referred to DSS. <br />% Each case viii be evaluated by DSS and referred appropriately to the <br />~1 Program. <br /> <br /> <br />
