Page 3 of 6 <br /> <br />9. DSS shall furnish the Purchaser ~ith monthly program report and request <br /> a <br /> for payment of units of service delivered. Such reports shall be furnished <br /> 3 working days after the end of each month. A client 101 Registration Form <br /> mst be submitted ~ith reports provided as a courtesy by the P~rchaser. <br /> <br />10. DSS shall encourage participants to make monthly contributions based on <br /> sliding fee scale. Contributions shall be mailed or delivered to the <br /> Cabarrns County Department of Aging. No participant is denied services due <br /> to inability to contribute. <br /> <br />11. Interest of Hembers of the ProKram and Others. No officer~ member, or <br /> amployee of the ProBram, and no public official of any local government <br /> which is affected in any way by the Program, ~ho exercises any function or <br /> responsibilities in the revie~ of the Pzogram or any component part thereof, <br /> shall participate in any decisions relatini to this contract ~bich affects <br /> his/her personal interest or the interest of any corporation, partnership or <br /> association in which he/she is, directly or indirectly, interested; nor <br /> any person any interest, or indirect, in <br /> have <br /> direct <br /> this <br /> contract or the proceeds arising therefrom. <br /> <br />12. Officials not to Benefit. No member of or deleBate to the Congress for the <br /> United States of Ar~erica, resident Cemissioner or employee of the United <br /> States Goverament, shall be entitled to any share or part of this contract <br /> or any benefits to arise herefrom. <br /> <br />13. Equal E~plo)nnent Opportunity. <br /> <br /> (a) DSS shall not discriminate against amployee, applicant for <br /> employment or subcontractor because of race~ color, handicap, religion, <br /> age, sex, or eational origin. DSS shall take affirmative action to <br /> insure that applicants are amployed and subcontractors (subcontractors <br /> mst he ~tuaily approved) are selected, and that employees are treated <br /> during employment, without regard to their race, color, handicap, <br /> religion, abe, sex or national origin. Such action shall include, but <br /> not be limited to the follo~ieg: employment, upgrading~ demotion, or <br /> transfers; recruitment or recruitment advertising; layoffs or <br /> terminations; rates of pay or other fo~ms of compensation selections <br /> for training including apprenticeship; and participation in <br /> recreational and educational activities. DSS certifies that it <br /> presently has in effect such an affirmative action program. DSS agrees <br /> to post in a conspicuous place available to employees and applicants <br /> for amploymeet, notices setting forth the provisions of this <br /> non-discrimination clause. DSS shall in all solicitations or <br /> aflvertisements for subcontractors or employees placed by or on behalf <br /> of DSS state'that DS~ in an "Equal Opportunity F~ployer". (b) DSS <br /> shall keep such records and submit such reports concerning <br /> characteristics of applicants for employment and employees as the <br /> Purchaser and the North Carolina Division of Aging may require. (c) <br /> DSS agrees to comply with such guidelines as the Purchaser or the North <br /> Carolina Division of Aging ~ay issue to implement the requirements of <br /> theparagraph. <br /> <br /> <br />
