Page 1 of ~ <br /> STATE OF NORTH CAROLINA ~H~NDMENT TO CONTRACT <br /> <br /> Amendment Number 2 to Contract, Contract for Services Provided for <br /> Under Title III of the Older Americans Act and/or Social Services Block <br /> Grant Funds and or North Carolina State Grant ~unds <br /> <br /> This ;tmendment, entered into this 15th day of May , <br /> 1991 , by and between the Centralina Council of Governments (hereinafter <br /> called the "CoUncil") and CabarFus County ffor services provided by its <br /> <br /> DeDartment of Aqinq) (hereinafter called the Agency"). <br /> <br /> WITNESSETH THAT; <br /> <br /> WHEREAS, the Council and the Agency entered into that certain agreement <br /> <br /> dated July 1, 19~0 pursuant to which the Agency was to <br /> <br />provide certain services for persons 60 years of age and older; and <br /> <br /> WHERE&S, certain changes may need to be made in projections of units of <br />service to be provided in order to recognize past performance as well as <br />needs for the remainder of the contract period; and <br /> <br /> Wa~REAS, certain changes may need to be made in the A3%A unit rates; and <br /> ka~EAS, certain changes may need to be made in the total grant funds; <br /> <br /> NOW, THEREFORE, it is agreed that the aforesaid Contract be and it is <br />hereby amended in the following respects: <br /> <br /> 1. Paragraph 2 of the Contract is amended by deleting therefrom the <br /> following services, number of units, AAA unit rates and total grant <br /> funds, <br /> <br /> <br />
